Must be licensed in the state of Florida.</p> <p>Purpose of Your Job Position</p> <p>The primary purpose of your job position is to coordinate nutritional care of residents by completing nutritional assessments, developing and implementing care plans, and documenting dietary information about residents, in accordance with current federal, state, and local standards, guidelines and regulations, and Solaris HealthCare's established policies and procedures.</p> <p>Job Functions</p> <ul> <li> <p>General Responsibilities</p> </li><li> <p>Residents Rights</p> </li><li> <p>Food Safety</p> </li><li> <p>Safety</p> </li><li> <p>Staff Development</p> </li><li> <p>Nutritional Assessment Responsibilities</p> </li><li> <p>Completes nutritional assessment, MDS, CAA's and care plans.</p> </li><li> <p>Completes progress notes.</p> </li><li> <p>Assesses and completes required documentation on residents nutritionally at risk according to time frames for assessments.</p> </li><li> <p>Visits residents routinely to determine effectiveness of interventions and to update food preferences and evaluate food intake.</p> </li><li> <p>Promotes and assists in implementation of liberalized diets.</p> </li><li> <p>Follows house supplement procedure recommending enhanced foods before supplements. Evaluates the effectiveness of supplements on a routine basis.</p> </li><li> <p>Reviews weight records routinely and communicates variances to interdisciplinary team.</p> </li><li> <p>Makes enteral feeding recommendations that are consistent with procedures.</p> </li><li> <p>Evaluates and monitors TPN.</p> </li><li> <p>Ensures timely follow-up on physician recommendations.</p> </li><li> <p>Communicates resident nutrition and hydration relevant changes in condition during the 24-hour Clinical Report/Stand-Up Meeting.</p> </li><li> <p>Uses current standards of dietetic practice in the implementation of nutritional care.</p> </li><li> <p>Serves as Dining Room Coordinator, Dining Room Monitor as required.</p> </li><li> <p>Oversees and ensures accuracy of nutritional reviews done by the Dietetic Technician and the Food Service Director.</p> </li><li> <p>Systems Responsibilities</p> </li><li> <p>Communication Responsibilities</p> </li><li> <p>Meal Service/Menus Responsibilities</p> </li><li> <p>Administrative Responsibilities</p> </li><li> <p>Special Activities/Attributes</p> </li></ul> <p>Education</p> <p>Registration with the Academy of Nutrition and Dietetics and licensed by the state where he/she practices, if appropriate.</p> <p>Experience</p> <p>At least one year experience in long term care, preferred.</p>
