<p>Our ImageCare Latham office is looking for a Breast Center Navigator! The hours are Monday-Friday 8:30am-5:00pm.</p> <p>The Breast Center Patient Navigator's primary function is to guide breast care for patients throughout the health system, assisting with access to providers and tracking the interventions and outcomes of their care.</p> <p>Compensation: $39.00 - $50.00 per hour</p> <p>Essential Functions:</p> <p>The primary role as the navigator for the Breast Center is to guide patients through the health care system, initiate and facilitate contact for biopsy appointments, surgical consultations or follow up care. This includes community and social support services.</p> <p>Additional Duties:</p> <p>The navigator will facilitate the interaction between the breast imagers and providers involved in the patients care.</p> <p>They are responsible for the tracking of patient's outcomes, and providing the results to the breast radiologist.</p> <p>Under the direction of the breast radiologist, the navigator relays results to the patients indicated by the radiologist.</p> <p>The navigator produces the required documents for each procedure performed.</p> <p>Environmental/Working Conditions:</p> <p>Ability to multitask, organize and communicate effectively. Fast-paced work environment. Answer phone calls and questions in a timely manner.</p> <p>Physical/Mental Demands:</p> <p>Ability to work at a computer for the length of your shift 8 - 10 hours. Answer phone calls and voicemail efficiently and effectively. Ability to change tasks when needed.</p> <p>Work closely with patients newly diagnosed with breast cancer and manage their care with professionalism and compassion.</p> <ul> <li>Licensure: Provide proof of valid medical license as a Registered Nurse or medical imager/radiologic technologist within the applicant's professional role and scope of practice. </li><li>Experience: Complete at least one year of breast care navigation practice (defined as 1500 hours) prior to certification testing. Experience must be within 2 years. </li><li>Strong computer skills </li><li>Ability to communicate effectively and work as a team </li><li>Must work independently and be self-motivated </li></ul>
