If you're passionate about preventative care, chronic disease management, and coordinated healthcare delivery, we want to hear from you.</p> <p>What You'll Do</p> <p>As an integral member of our VBC team, you'll:</p> <p>Provide Direct Patient Care:</p> <ul> <li>Conduct Annual Wellness Visits (AWVs) focused on prevention, risk assessment, and advance care planning for Medicare patients. </li><li>Manage chronic and acute conditions through evidence-based care plans. </li><li>Close care gaps with focused visits that address quality metrics and improve health outcomes. </li></ul> <p>Coordinate and Lead Care Efforts:</p> <ul> <li>Triage patient needs via phone and electronic messages. </li><li>Collaborate closely with primary care providers, specialists, care managers, and pharmacists. </li><li>Guide and support medical assistants and care coordinators in clinical efforts. </li></ul> <p>Drive Quality and Performance:</p> <ul> <li>Monitor and improve key performance indicators such as HEDIS and Stars metrics. </li><li>Participate in strategic meetings to optimize workflows and care models. </li><li>Lead efforts in patient outreach, population risk stratification, and process improvement. </li></ul> <p>Ensure Excellent Documentation and Compliance:</p> <ul> <li>Maintain thorough and timely documentation in the EMR. </li><li>Support accurate coding and regulatory compliance for VBC requirements. </li></ul> <p>Empower and Educate Patients:</p> <ul> <li>Provide personalized education on managing chronic illnesses, medications, and preventative care. </li><li>Encourage patient engagement and shared decision-making. </li></ul> <p>What We're Looking For</p> <p>Education & Licensure:</p> <ul> <li>Master's or Doctorate in Nursing (NP) or completion of an accredited PA program. </li><li>Unrestricted Utah license and national certification (AANP, ANCC, or NCCPA). </li><li>DEA licensure and BLS certification required; ACLS preferred. </li></ul> <p>Experience:</p> <ul> <li>Minimum 2 years as an APP preferred. </li><li>Background in primary care, geriatrics, or chronic disease management strongly preferred. </li><li>Value-Based Care or population health experience a major plus. </li><li>EMR proficiency required; experience with systems like Epic, Cerner, or Athenahealth is ideal. </li></ul> <p>Key Skills:</p> <ul> <li>Clinical excellence in assessment, diagnosis, and treatment. </li><li>Strong communication and patient education abilities. </li><li>Team-oriented with leadership capabilities. </li><li>Deep understanding of value-based care and quality metrics. </li></ul> <p>Why Revere Health?</p> <ul> <li>Collaborative Culture: </li></ul> <p>Work alongside passionate professionals committed to improving patient lives.</p> <ul> <li>Innovative Model: </li></ul> <p>Be a part of cutting-edge initiatives that prioritize outcomes over volume.</p> <ul> <li>Meaningful Mission: </li></ul> <p>Contribute to a cause bigger than a paycheck-changing the future of healthcare.</p> <p>Ready to Make a Difference?</p> <p>Apply today and join a team that's redefining healthcare in Utah.
