Navigation services include Prevention Navigators engaging in motivational interviewing to assist clients with goal setting, reducing barriers to care and treatment, and longer-term interventions to refer clients to community partners and services. External referral connections may include but are not limited to medical and STI care, social services such as housing, employment, insurance enrollment, and transportation assistance, and behavioral health and AODA treatment services.</p> <p>The Navigation Supervisor will create and implement systems designed to facilitate and monitor prevention and navigation services while performing administrative activities satisfying guidelines established by both Vivent Health and various funding agencies. This position also oversees the development, management, and oversight of prevention staff.</p> <p>Essential Functions</p> <ul> <li> <p>Implement Prevention Navigation and harm reduction programming, including supervising program staff. Addressing, teaching, and monitoring the following:</p> </li><li> <p>Client data collection including assessments and risk reduction planning.</p> </li><li> <p>Documentation monitoring and reporting.</p> </li><li> <p>Ongoing client continuity of care monitoring, including enabling and tracking access, linkage, and retention to services.</p> </li><li> <p>Community referral network development and maintenance.</p> </li><li> <p>Program design, planning, and implementation of Prevention Navigation Services.</p> </li><li> <p>Development and oversight of client focus groups, surveys, and evaluation activities.</p> </li><li> <p>Implement state of the art HIV prevention programs targeting those at highest risk for HIV, including people who use drugs, in current and emerging markets.</p> </li><li> <p>Train and supervise program staff at both Vivent Health offices and in the community. Skilled at both in-person and virtual management.</p> </li><li> <p>Provide staff training, development, and ongoing monitoring to ensure implementation of standardized services and compliance with state and grant requirements.</p> </li><li> <p>Effectively manage financial budgets, including preparation of budgets, review and approval of expenditures, invoice submission, budget/expenditure reconciliation, and supply orders in accordance with established agency guidelines.</p> </li><li> <p>Collaborate with multiple internal and external stakeholders for submission of grant deliverable reports.</p> </li><li> <p>Fully responsible for the accuracy and timeliness of all required tasks, including budgets, reports, and supervision.</p> </li><li> <p>Responsible for recruitment and engagement of clients in prevention programming, including developing recruitment materials and initiatives for internal and external audiences, in collaboration with Marketing and Communications.</p> </li><li> <p>Effectively manage and coordinate grant deliverables, including but not limited to; develop and implement standard operating procedures, coordinate activities with other Vivent Health staff, create quarterly and annual reports (as required by funder), and utilize collected data to develop program reports and presentations for funding and marketing purposes.</p> </li><li> <p>Continuously research programs working with similar populations to ensure the target population is receiving the latest effective interventions available.</p> </li><li> <p>Network with community stakeholders including community-based organizations and HIV-related government agencies.</p> </li><li> <p>Collect and run productivity, compliance, and access to care reporting in order to monitor adherence to state standards, grant deliverables, and internal goals and outcomes. Review each team member's monthly performance and the team's progress towards retention and client linkage to care.</p> </li></ul> <p>Qualifications</p> <ul> <li>Bachelor's degree or four (4) years of related work experience in an environment dealing with prevention, navigation, program development, data reporting and funding. </li><li>1+ years of experience with program and partnership development, case management, community engagement, and/or staff training. </li><li>3+ years of supervisory experience. </li><li>Must have valid driver's license. </li><li>Ability to complete work assignments with minimal supervision, thoroughness and efficiency. </li><li>Experience in starting or launching a program from the ground up. </li><li>History of allyship and advocacy in a social work or public health-related field. </li><li>Experience providing harm reduction services. </li><li>Ability to communicate, network effectively, and work cooperatively with individuals from diverse cultural, ethnic, and social backgrounds. </li><li>Experience with preparing grant proposals, writing reports, and managing budgets. </li><li>Experience with community development, engagement, and advocacy. </li><li>Knowledge of harm reduction and related medical, social service, mental health, and addiction treatment delivery systems. </li><li>Working knowledge of Spanish language (bilingual) is a plus. </li></ul> <p>Salary:</p> <p>Starting at $51,000/annually (Depending on Experience)</p> <p>Public Student Loan Forgiveness</p> <p>Employment at our organization may qualify you for federal student loan forgiveness programs.
