Currently, we are hiring for High Fidelity Wraparound Facilitator.</p> <p>Responsibilities</p> <p>Ideal candidate lives in Swain, Macon, Jackson or Haywood county</p> <p>The High Fidelity Wraparound Facilitator will engage youth and families in developing, implementing, and monitoring an intensive, individualized care plan. This includes helping the team develop a natural support system, working on the development of an integrated plan and establishing a strong relationship with the family.</p> <p>Position Overview</p> <ul> <li>Carry small caseload of 10-12 families </li><li>Facilitate High Fidelity Wraparound services: utilize a strengths-based approach to safety planning, development of family team and family support systems, and wraparound planning for the purpose of maintaining children in their homes, schools, and communities </li><li>Prepare Wraparound Plan with families and agency partners with measurable outcomes </li><li>Complete documentation in an electronic health record </li><li>Become credentialed as a High-Fidelity Wraparound Facilitator within 12 months of employment </li></ul> <p>Ideal Candidate</p> <ul> <li>Passionate about applying the principles of High-Fidelity Wraparound, including Family Voice & Choice, Team Based Planning, Natural Supports, Collaboration, Strengths Based approach, and more </li><li>Experience providing services for children and families with complex needs </li><li>Experience facilitating Child and Family Teams </li><li>Experience with case management </li><li>Experience with Person Centered Thinking and Systems of Care (SOC) </li><li>Knowledge of community resources; preferably experience with local resources </li><li>Understanding and desire to collaborate with different child-serving systems, including schools, mental health, child welfare, juvenile justice, health and others </li></ul> <p>Salary</p> <p>$58,000 - $66,000 / year based on education level and clinical license</p> <p>Qualifications</p> <p>Position Requirements</p> <p>High Fidelity Wraparound Facilitators must meet the qualified professional standard for North Carolina. Please refer to the requirements below to ensure that your education and experience meet the qualifications.</p> <p>Bachelor's Degree Applicants:</p> <ul> <li>Possess a degree in a related field of study (Social Work, Counseling, Psychology, Marriage and Family Therapy)*-Have two years (pre- OR post-graduation) of experience (4,000 hours) with at-risk youth and families. Experience should include clinical case management or therapeutic interventions. </li></ul> <p>Master's Degree Applicants:</p> <ul> <li>Possess a degree in a related field of study (Social Work, Counseling, Psychology, Marriage and Family Therapy)*-Have one year (pre- OR post-graduation) of experience (2,000 hours) with at-risk youth and families.
