Your recruiter will provide the most up-to-date details during the hiring process.</p> <p>How you'll make an impact in this role</p> <ul> <li>Deliver evidence-based individual and group counseling to empower patients in achieving optimal mental health outcomes and personal growth. </li><li>Assess patient needs and establish personalized, measurable treatment goals to guide the therapeutic journey toward sustained recovery. </li><li>Collaborate actively with the patient, family, medical team, and community providers to ensure coordinated, comprehensive care from the initial assessment through discharge. </li><li>Evaluate the effectiveness of therapy interventions regularly, adjusting treatment plans collaboratively with the patient to drive positive and sustained progress. </li><li>Document patient care journeys with thorough, confidential, and high-quality records to ensure continuity of care and accurate treatment tracking. </li></ul> <p>What minimum requirements you'll need</p> <p>Licensure / Certification / Registration:</p> <ul> <li> <p>One or more of the following required:</p> </li><li> <p>Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.</p> </li><li> <p>Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.</p> </li><li> <p>Social Worker specializing in Clinical Practice credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.</p> </li><li> <p>Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.</p> </li><li> <p>Therapist specializing in Marriage & Family credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.</p> </li><li> <p>BLS Provider preferred.
