<p>Join our Home Care team at The Cypress of Charlotte as an RN Case Manager within independent living, where you'll lead patient care coordination, ensuring quality and compliance for our home care clients. This role involves conducting assessments, creating and revising care plans, and providing skilled nursing visits, all while maintaining strong communication with clients, physicians, and our team.</p> <p>Key Responsibilities:</p> <ul> <li>Conduct initial assessments and create personalized care plans for new independent living clients. </li><li>Ensure all care plans are reviewed and updated every 90 days, in compliance with NC home care regulations. </li><li>Make in-home skilled nursing visits in accordance with physician orders, focusing primarily on medication management. Provide direct nursing care, performing skills safely and in compliance with regulations. </li><li>Maintain clear, accurate documentation in eRSP system and ensure timely updates on all care plans. </li><li>Collaborate with the Home Care Director and team to ensure seamless care delivery. </li><li>Participate in on-call shifts and after-hours support as needed. </li><li>Promote Cypress's culture of hospitality by treating all clients, staff, and visitors with respect and care. </li><li>Ensure a holistic approach to wellness for both residents and employees. </li></ul> <p>Qualifications:</p> <ul> <li>Valid NC RN license (or compact) with no history of abuse or neglect. </li><li>BSN preferred. </li><li>Minimum of 2 years of nursing experience preferred but not required. </li><li>Experience with senior population desired. </li><li>Ability to work on-call rotations with Home Care team. </li><li>Familiarity with NC Home Care regulations. </li><li>Proficiency in electronic charting and MS Office. </li><li>Strong organizational, communication, and time management skills. </li><li>Ability to work both independently and as part of a collaborative team. </li></ul> <p>Wondering if this position is for you?
