<p>Description</p> <p>CCAP Mission Statement - To empower all people and communities, challenged by poverty as well as social and cultural barriers, through advocacy, education, and access to high quality health and human services.</p> <p>JOB SUMMARY:</p> <p>The Integrated Behavioral Health Clinician is embedded within the primary care teams and works directly with the teams to assess individuals with mental health and/or substance use challenges and provides brief treatment services within the primary care setting. The Integrated Behavioral Health Clinician's goals are to help improve recognition, treatment, and management of behavioral health and medical conditions in the population. The Integrated Behavioral Health Clinician applies the biopsychosocial model to the primary care setting and demonstrates a basic knowledge of pharmacotherapy. The Integrated Behavioral Health Clinician works as a consultant to the primary care teams.</p> <p>Requirements</p> <p>REQUIRED QUALIFICATIONS:</p> <ul> <li>Must hold a master's degree in Social Work or related field from an accredited college; preferred </li><li>Licensed Clinical or Independent Clinical Social Worker in the State of Rhode Island; preferred </li><li>Previous experience or internship in a medical setting which supports a primary care integration model; preferred </li><li>Provide/maintain all required immunizations and/or vaccinations </li><li>Complete all required background checks </li></ul> <p>WORK SCHEDULE DEMANDS:</p> <ul> <li>Full-time; evening work may be required </li><li>Part-time when decided by the director or VP of program. </li><li>Up to two evenings may be required. </li><li>May be required to work at any CCAP component/location and may be required to work in the community. </li><li>Due to the nature of the work and level of responsibility, the individual maintains a flexible schedule. </li></ul> <p>KEY RESPONSIBILITIES:</p> <p>Clinical Screening:</p> <ul> <li>The following responsibilities are not meant to be all-inclusive and may be expanded or adjusted to meet component needs </li><li>Provides adult and adolescent behavioral health screening within a primary care setting (family health) </li><li>Specifically administers GRPA screens within a primary care setting and utilizes all behavioral health screens as necessary to provide comprehensive screening services for health setting clients/patients </li><li>Participates in team "huddles" within the primary care setting(s) in support of the medical home model </li><li>Utilizes the electronic health record to document all client/patient encounters </li><li>Develops and maintains good working relationships with internal/area community health providers, schools, legal, welfare, and other social/human service and mental health personnel and agencies </li><li>Provides case-management services as necessary </li><li>Conducts workshops, in-services, and educational programs to other CCAP components and the community specific to the S-Birt model of integrated care </li></ul> <p>Administrative:</p> <ul> <li>Completes all written work: screening summaries, reports in a timely manner </li><li>Participates in weekly supervision (group and/or individual) </li><li>Manages the clinical record in accordance with CCAP policies and procedures </li><li>Participate in performance improvement committees or other committees as assigned </li><li>Must maintain an agency established benchmark of number of screens delivered per week </li><li>Participates in community boards, groups, and councils as required </li><li>Participates and assists in departmental studies, projects as required </li><li>Participates in professional responsibilities and activities including departmental statistical generation and analysis, program development </li><li>Participates in trainings required by CCAP and other trainings necessary for the maintenance of professional licensure and/or supervisory requests for growth and improvement </li><li>Other duties as assigned </li></ul> <p>CONFIDENTIALITY OF INFORMATION:</p> <p>Has unrestricted access to clinical records assigned to caseload. Must follow State and Federal Laws of confidentiality of Clinical Records and must follow CCAP policies and procedures for the creation, protection, retention, release, and disposal of Clinical Records. All staff are expected to demonstrate cybersecurity awareness and adhere to best practices. This includes familiarity with phishing emails, creating strong passwords, and participating in regular cybersecurity training sessions.</p> <p>PHYSICAL EFFORT/ENVIRONMENT:</p> <p>The physical demands and work environment described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
