<p>Position Summary</p> <p>Primary function is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family and referring agency.</p> <p>Essential Duties and Responsibilities</p> <ul> <li>Under the direction of the RN, assist in identifying the patient's physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations. </li><li>Participate in planning and implementing care in conjunction with the RN, in accordance with the POC. </li><li>Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease. </li><li>Assure the continuity of care through delivery of quality patient care. </li><li>Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations. </li><li>Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations. </li><li>Demonstrate commitment, professional growth and competency. </li><li>Promote Agency philosophy and administrative policies to ensure quality of care. </li></ul> <p>Knowledge, Skills and Abilities</p> <ul> <li>Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. </li><li>Reliable transportation. Valid and current auto liability insurance. </li><li>Works in patient's home in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather. </li><li>Physical skills </li><li>Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. </li><li>Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition. </li><li>Meet patient/family individualized psycho social needs. </li><li>Requires hand-eye coordination and manual dexterity. </li></ul> <p>Education & Experience</p> <ul> <li>Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC, current driver's license. </li><li>Graduate of an accredited School of Vocational/Practical Nursing. </li><li>Two years experience as a Licensed Vocational/Practical Nurse in a clinical care setting, hospice preferred. </li></ul>
