<p>Job Summary and Responsibilities</p> <p>As our Case Manager RN, you will meticulously coordinate patient care and services across the entire illness continuum. You will ensure the effective utilization and judicious monitoring of healthcare resources, and collaboratively lead the interdisciplinary team to achieve superior patient outcomes through integrated case management practices.</p> <p>Every day you will perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, and development of a discharge plan.
