We are a trusted, family owned, Medicare certified Home Health agency with almost 20 years of experience providing a wide range of skilled and nonskilled services across the Middle Tennessee area.</p> <p>Our Mission:</p> <p>Our mission at All Heart Home Care is to be a cost effective, quality provider of home health services that promote and enhance the quality of life for those we serve.</p> <p>The Admissions Coordinator Position:</p> <p>This position will train out of the Nashville office. Once training is complete, the employee will have the option to work remotely three days out of the week. The Admissions Coordinator duties will include answering phone calls, taking messages, and providing information to patients and their families regarding our mobile wound care services, coordinating intake processes for new patients, including completing new patient paperwork and collecting necessary consents to treat, Cross-training across company brands to verify insurance coverage and manage approvals and prior authorizations to facilitate patient care. Additionally, communicate any out-of-pocket costs to the patient and obtain informed financial consents, Scheduling appointments for initial assessments and follow-up visits, ensuring efficient coordination of care, Conducting interviews with patients over the phone to gather demographic information, confirm living situations, and assess social support systems, reviewing medical records and other documents to assess patients' needs and determine eligibility for home health care services, Serving as a liaison between patients and healthcare providers, coordinating patient care plans with nurses, nurse practitioners, medical doctors, and other healthcare team members, communicating effectively (both written and orally) and maintain professional relationships with caregivers, medical staff, patients, and their families, maintaining and reporting various administrative records and counts to supervisors, including complaints and resolutions, admissions, discharges, referral sources, and patient locations, building and maintaining relationships with surrounding medical agencies for potential referrals, conducting all activities with the highest standards of professionalism and confidentiality, in compliance with Medicare and Medicaid regulations, supporting quality improvement programs and participate in continued education and staff training, and following Infection Control Policies in all settings.</p> <p>The Admissions Coordinator needs to have previous experience with intake, insurance verification or transitional care. Must have familiarity with medical software and equipment. Must demonstrate a high commitment to customer service and quality standards. Prior experience in Home Health/Hospice services is preferred.
