Coordinate the plan of care. Utilize population-based tools to support and monitor wellness and care goals for each patient, aimed at preventing illness and improving individual well-being, clinical outcomes and quality of</p> <p>life while managing multiple chronic illness conditions. Coordinate care support self-management of patients with complex, chronic medical and psychosocial issues.</p> <p>Utilizes current nursing practices to provide patient care while focusing on the needs of the patient and exemplifying the service excellence goals of care, concern, respect and cooperation. Preferred: Master's Degree</p> <p>Experience Required: 6-10 years of previous job-related experience with 3-5 years in sub-acute, acute inpatient rehabilitation or similar setting.</p> <p>Experience Details: Ambulatory or Case Management Experience: 1-2 years</p> <p>Licensure/Certifications: Required: Current Registered Nurse License State of Illinois.
