<p>Title: Registered Dietitian (RD/RDN)</p> <p>Location: New Haven, CT.</p> <p>Cornell Scott-Hill Health Center is looking add a Full Time Registered Dietitian to our Nutrition team!</p> <p>The Registered Dietician is responsible for providing high-quality, culturally competent nutrition services and diabetes education to CS-HHC patients through individual counseling sessions, teaching self-management classes, and by providing group presentations. Assist in the promotion of nutrition services throughout CS-HHC, maintain accurate records of nutrition encounters, maintain evidenced-based current educational materials, and assist in the development and implementation of cost-effective nutrition interventions that more closely meet the nutritional needs of patients. There may be an opportunity for remote work.</p> <p>Responsibilities include but are not limited to:</p> <ul> <li>Provide individual nutrition counseling tailored to the specific needs of each patient, with particular attention and sensitivity to the cultural food practices of an ethnically diverse population. </li><li>Teach classes to assist and educate patients in self-management and monitoring of diabetes, weight management, and other disease focused dietary needs. </li><li>Assist in development of diabetes prevention programing, general curriculum, and educational tools. </li><li>Maintain accurate and up-to-date records of nutrition services through approved EMR, including timely completion of documentation and billing requirements </li><li>Work well independently and with interdisciplinary team, including medical and support staff, to provide services and prioritize patient care. </li><li>Maintain status as a Registered Dietitian by regularly participating in approved CEU activities. </li><li>Participate in peer review process. </li><li>Performs all other duties as required. </li></ul> <p>Requirements:</p> <ul> <li> <p>Education:</p> </li><li> <p>Bachelor's Degree in food and nutrition, dietetics, nutritional sciences, or human nutrition is required.</p> </li><li> <p>RD/RDN credentialing and active status through Commission on Dietetic Registration. State Certification.</p> </li><li> <p>If Certified Diabetes Educator: CDE certification. Credentialing through the Certification Board for Diabetes Care and Education</p> </li><li> <p>Experience:</p> </li><li> <p>Completion of an accredited dietetic internship.</p> </li><li> <p>Experience and training in individual outpatient nutrition care and group education.</p> </li><li> <p>Certified Diabetes Educator (CDE/DCES) preferred. Experience counseling patients in diabetes self-monitoring of blood glucose (SMBG), and teaching DSME (Diabetes Self-Management Education).</p> </li></ul> <p>CS-HHC is an equal-opportunity employer.
