<p>Little Flower Children and Family Services of New York is seeking a Service Provider to provide Children and Family Treatment and Support Services and Home and Community Based Services to enrolled children.</p> <p>Since 1930, Little Flower has been committed to improving the lives and well-being of those we serve by providing foster boarding home care, residential treatment care and where appropriate, adoption. Our work focuses on strengthening the family so that they can provide a safe nurturing environment for raising children and to overcome a myriad of obstacles that threaten a child's safety.</p> <p>Home and Community Based Services (HCBS) and Children and Family Treatment and Support Services (CFTSS) offers opportunities to better meet the behavioral health needs at earlier junctures in a child/youth's life to prevent the onset or progression of behavioral health conditions. These services are also intended to help prevent the need for more restrictive and higher intensity services for children and youth.</p> <p>The CFTSS Service Provider is responsible for providing the services of Community Psychiatric Support (CPST), and Psychosocial Rehabilitation (PSR) to qualifying children and families. The HCBS provider is responsible for providing the services of Caregiver Family Supports and Services, Prevocational Services, Supportive Employment Services, Planned Respite, Community Habilitation and Community Self- Advocacy Training and Support. Services are provided directly to the child and/or the family and will often take place in home or community settings.</p> <p>Service Providers will also provide CPST and PSR during a child's visitation with biological parents/families. Service Providers will utilize the principals and best practices of Visit Coaching to deliver effective services during these family time opportunities.</p> <p>Principle Responsibilities:</p> <ul> <li>Participation in the development and ongoing monitoring of the child's Treatment Plan. </li><li>Provide direct services to child & family on caseload. </li><li>Documentation of all service provision and contacts. </li><li>Attendance at all meetings deemed necessary. </li><li>Coordination of care with other providers and/or collaterals to assist child in obtaining their optimal functioning level within the community setting. </li><li>Will report to Hauppauge office on an as needed basis for supervision and meetings. </li><li>Attend all required trainings. </li></ul> <p>Minimum Requirements:</p> <ul> <li>Bachelor's degree in Social Work, Psychology or a related field, plus two years of related experience required. </li><li>Master's degree in Social Work, Psychology or a related field preferred. </li><li>Excellent writing, communication and word processing skills required. </li><li>Ability to travel to various communities. </li><li>Ability to work a flexible schedule including weekends and evenings. </li><li>Possession of a valid driver's license is required. </li></ul> <p>Travel Requirements:</p> <ul> <li>Ability to travel to our Rockaway site </li></ul> <p>Disclaimer:</p> <p>The statements herein are intended to describe the general nature and level of work being performed by the incumbent in this position.
