<p>Case Manager</p> <p>Department: Adult Services Reports To: Director of Person-Centered Supports FLSA Status: Non-Exempt</p> <p>Position Summary</p> <p>The Case Manager (CM) plays a vital role in advocating for individuals served through a holistic, person-centered approach. This position oversees the Person-Centered Planning (PCP) process, ensures service compliance, and acts as a liaison between NCIA, LTSS systems, and state agencies. The CM ensures that services are aligned with each individual's goals, while monitoring funding and addressing gaps to support long-term success and independence.</p> <p>Mission-Driven Impact</p> <p>NCIA is a nonprofit organization committed to empowering individuals to reach their fullest potential. The Case Manager supports this mission by promoting inclusive, person-centered services, ensuring regulatory compliance, and fostering collaborative relationships that drive meaningful outcomes.</p> <p>Key Responsibilities</p> <p>Person-Centered Planning & Advocacy</p> <ul> <li> <p>Lead and facilitate the PCP process, ensuring implementation, training, and compliance</p> </li><li> <p>Advocate for services that reflect the goals and preferences of individuals served</p> </li><li> <p>Coordinate PCP meetings with CCS 90-180 days before due dates</p> </li><li> <p>Ensure annual goals are measurable and supplemental assessments are completed</p> </li><li> <p>Review PCP packets for accuracy and compliance</p> </li><li> <p>Ensure Community Setting Rule requirements are met</p> </li></ul> <p>Service Monitoring & Compliance</p> <ul> <li> <p>Review and update service plans weekly in LTSS/ICM software</p> </li><li> <p>Monitor LTSS to verify funding and authorize services based on approved PCPs</p> </li><li> <p>Save PCPs in ICM and assist with staff training</p> </li><li> <p>Notify Fiscal Department and program directors of funding changes</p> </li><li> <p>Communicate service start/end dates to teams</p> </li><li> <p>Support corrective action plans in response to OHCQ findings</p> </li></ul> <p>Relationship Building & Communication</p> <ul> <li> <p>Conduct quarterly home visits for Community Living participants</p> </li><li> <p>Make monthly calls to families and guardians of Day Program and Business Services participants</p> </li><li> <p>Complete bi-monthly follow-along visits with individuals working in the community</p> </li><li> <p>Maintain cooperative communication with staff, supervisors, families, and external partners</p> </li></ul> <p>General Responsibilities</p> <ul> <li> <p>Maintain regular attendance per agency policy</p> </li><li> <p>Perform other duties as assigned to support NCIA's mission</p> </li></ul> <p>Supervisory Responsibilities</p> <p>This position does not include direct supervision of staff.</p> <p>Qualifications</p> <p>Education & Experience</p> <ul> <li> <p>Bachelor's degree in social work, counseling, psychology, special education, or related field</p> </li><li> <p>Minimum 2 years of experience working with individuals with intellectual or developmental disabilities</p> </li><li> <p>Minimum 2 years of experience in case management and service plan development</p> </li><li> <p>Reliable transportation and acceptable driving record per agency and insurance requirements</p> </li></ul> <p>Certifications & Skills</p> <ul> <li> <p>Must obtain and maintain current MANDT and CPR/First Aid certifications</p> </li><li> <p>Excellent writing and meeting facilitation skills</p> </li><li> <p>Ability to read and interpret instructions, memos, and correspondence</p> </li><li> <p>Strong interpersonal and customer service skills</p> </li><li> <p>Proficient in Microsoft Office Suite and database systems</p> </li><li> <p>Solid organizational, time management, and multi-tasking abilities</p> </li><li> <p>Ability to meet deadlines and use technology for job-related tasks</p> </li></ul> <p>Why NCIA?</p> <p>NCIA is a mission-driven organization committed to transforming lives through opportunity and support.
