<p>Position Summary:</p> <p>The Case Manager provides supportive services to high-risk individuals through assessments, individualized planning, and service coordination. This role ensures participants access to housing, employment, behavioral health, and trauma recovery support.</p> <p>Job Responsibilities:</p> <ul> <li>Complete intakes and eligibility documentation. </li><li>Develop and implement individual case plans with measurable goals. </li><li>Maintain a caseload of up to 25 participants. </li><li>Provide referrals to behavioral health, housing, job training, and legal resources. </li><li>Track outcomes and update participant files with progress notes. </li><li>Attend interdisciplinary team meetings and case conferences. </li><li>All other duties as assigned. </li></ul> <p>Education, Experience, and Skills Required:</p> <p>Education: Bachelor's degree in Social Work, Psychology, Human Services, or related field.</p> <p>Experience: Two years of experience in case management or violence prevention work.</p> <p>Skills:</p> <ul> <li>Strong documentation and time management skills. </li><li>Effective communication and interpersonal abilities. </li><li>Trauma-informed and culturally competent approach. </li></ul> <p>Location:</p> <p>In-person/On-site; both in IL and MO</p> <p>At BHR, we believe that every team member has an integral role in the lifesaving treatment we provide.
