<p>Day Kimball Health is hiring for a per diem Physical Therapist to join our Inpatient Physical Medicine department in Putnam, CT!</p> <p>Location: Putnam, CT</p> <p>Schedule: Per Diem (All shifts)</p> <p>Per Diem Requirement: Per diem employees are expected to work a minimum of two 8-hour shifts (16 hours total) per month. Join us in our mission to elevate community-driven healthcare and be part of an organization that values both personal and professional development.</p> <p>Physical Therapist Job Summary:</p> <p>As the Physical Therapist (PT), under the general direction of the Physical Medicine Director, you will organize and administer prescribed physical therapy programs in an effort to reduce and/or prevent disability to help patients achieve their maximum functional potential. You will also discover a culture of teamwork, professionalism, and mutual respect, and-most importantly-a fulfilling career.</p> <p>Physical Therapist Job Responsibilities:</p> <ul> <li>Evaluates and treats patients as prescribed by referring physician. </li><li>Records treatment, response, progress and discharge information in patient's chart or enters information into computer. </li><li>Educates patient, family and significant others regarding condition, treatment and home program. </li><li>Routinely confers with physician and other practitioners in person, by phone, or in writing, regarding progress, regression or change of plan. </li><li>Orients, instructs, and directs assistants, aides and students. </li><li>Maintains professional growth by attendance of department meetings, inservice and educational conferences. Includes participation in hospital orientation and departmental and community inservice education and programs as directed. </li><li>Assists with the planning and implementation of new programs. </li></ul> <p>Physical Therapist Education and Experience Requirements:</p> <ul> <li>BS, MS or DPT degree from an accredited program in Physical Therapy required. </li><li>Licensure by the state of Connecticut required. </li><li>Current AHA Healthcare Provider CPR certification or equivalent within 3 months of hire required. </li><li>Minimum of one-year clinical experience preferred. </li></ul> <p>Physical Therapist Knowledge, Skills, and Abilities:</p> <ul> <li>Ability to review physicians' referral, patients' condition, and medical records to determine physical therapy treatment required. </li><li>Demonstrate skills in order to perform tests and measure patients' strength, motor development, sensory perception, functional capacity, along with respiratory and circulatory efficiency. </li><li>Plan and prepare written treatment program based on evaluation of patient data with realistic, objective, long and short-term goals and time frames. </li><li>Records treatment, response, progress and discharge information in patients' chart or enters information into computer, in measurable and objective terms as dictated by department policy. </li><li>Knowledge to administer individualized programs of physical therapy as determined by the physicians' order and the initial evaluation.
