<p>Job Summary and Responsibilities</p> <p>JOB SUMMARY / PURPOSE</p> <p>The Clinical Pharmacist Ambulatory Services provides direct patient care in hospital or community-based</p> <p>clinics. The pharmacist works collaboratively with physicians, nurses, other healthcare providers, and</p> <p>patients to optimize drug therapy and outcomes using evidence-based standards of care. This job is</p> <p>responsible for providing effective, efficient and accurate ambulatory pharmaceutical care to achieve</p> <p>organizational, patient care, and quality measure goals and objectives. The incumbent is responsible for</p> <p>working within the scope of their license, performed in accordance with organizational protocols,</p> <p>internal guidelines/standards, and any applicable State regulatory requirements.</p> <p>ESSENTIAL KEY JOB RESPONSIBILITIES</p> <ol> <li>Participates in the evaluation and management of diseases and health conditions in collaboration with </li></ol> <p>other health care providers</p> <ol start="2"> <li>Prescribe pharmaceutical agents as outline in the approved collaborative drug therapy management </li></ol> <p>agreement including initiating, adjusting, and discontinuing drug therapy</p> <ol start="3"> <li> <p>Order and interpret drug therapy-related tests</p> </li><li> <p>Ensure patients have access to and receive safe, efficacious and cost-effective medication therapy</p> </li><li> <p>Work collaboratively to resolve medication access and adherence issues</p> </li><li> <p>Address medication-related issues to support care coordination and transitions of care</p> </li><li> <p>Performs appropriate Point-of-care laboratory testing as required for the visit</p> </li><li> <p>Provides patients and/or families with focused teaching on safe and effective use of prescribed</p> </li></ol> <p>medications and non-pharmacologic management of chronic health conditions</p> <ol start="9"> <li>Precepts Pharmacy Interns and Pharmacy Residents. May serve as the Residency Program Director or </li></ol> <p>Coordinator if applicable to their location.</p> <ul> <li>Remote Position* </li></ul> <p>Job Requirements</p> <p>MINIMUM QUALIFICATIONS</p> <p>Required Education and Experience Graduation from an accredited school of pharmacy</p> <p>Preferred Education and Experience:</p> <p>Pharm.D. with:</p> <p>? Completion of three (3) years of practice experience with at</p> <p>least 50% of time spent in ambulatory care activities, or</p> <p>? Completion of a PGY2 Ambulatory Care Pharmacy residency, or</p> <p>? Completion of a PGY1 residency plus one (1) year of practice</p> <p>experience with at least 50% of time spent in ambulatory care</p> <p>activities</p> <p>Required Licensure and Certifications: Active Pharmacist License in good standing in the state(s) in which</p> <p>the employee works. PHARM:KY</p> <p>Required Minimum Knowledge, Skills,</p> <p>Abilities and Training:</p> <p>? Current knowledge of evidence based clinical pharmacy</p> <p>practice and pharmacotherapies</p> <p>? Knowledge of Health Plans such as Medicare, Medicaid, and</p> <p>commercial plans</p> <p>?
