<p>SUMMARY: The Healthy Transitions Case Manager will provide community-based outreach and case management to youth and young adults ages 16-25 with significant mental health concerns. Healthy Transitions is designed to assist clients with severe mental health diagnoses and co-occurring substance use with meeting personal objectives for improving mental health, obtaining employment, reaching educational goals, and making successful transition to adulthood. The Case Manager will be responsible for 1) Providing comprehensive care and coordination of services for clients and families enrolled in the program; 2) Implementing treatment plan and track progress of clients toward their goals; 3) Work collaboratively with Healthy Transitions team to promote, improve and sustain the overall wellness and physical health of the target population. The core Success Factors include: Instill Trust and Value Differences Patient and Community Focus and Collaborate RESPONSIBILITIES: RESPONSIBILITIES:•Maintain knowledge of the Healthy Transition's Coordinated Specialty Care evidence-based treatment model.•Provide case management interventions such as advocacy, resource linkage, skills building, support, and interventions to clients in their home and community•Assist clients in accessing eligible benefits or services, developing social and natural support network, applying skills in real life environments•Function as a liaison for client with Healthy Transitions team, community agencies, schools, employment sites and other providers.•Support clients as they work towards their employment and educational goals•Attend coordinated care meetings and report issues raised to the HT Team Leader.•Use creative approaches and Motivational Interviewing to engage clients who may be ambivalent about participating in treatment.•Provide individual and group interventions as identified in the treatment plan, in the home, at a Center facility, and in the community.•Assist clients in managing prescribed medications, supporting them in attending appointments, maintaining medication supply, monitoring side effects, and communicating with prescribers.•Coordinate care with other providers as prescribed in the treatment plan.•Maintain ongoing and collaborative relationship with client, team members, and community providers and partners•Maintain knowledge of available providers and resources in the community to address specialized needs.•Participate in case consultation, collaborative care meetings, and integrated treatment planning.•Attend agency and Healthy Transitions staff meetings as scheduled.•Attend weekly supervision as scheduled.•Maintain detailed, accurate, and timely documentation in medical record. MINIMUM QUALIFICATIONS: QUALIFICATION REQUIREMENTS:•Associate's degree, required. Bachelor's degree, preferred.•At least 2 years experience working with adolescents or young adults in a mental health or school settingNECESSARY SPECIAL REQUIREMENTS:•Knowledge of adolescent development and family systems issues.•Knowledge of community resources for clients and families with behavioral health disorders.•Advocacy and teaching skills.•Valid driver's license and use of car during normal working hours or other means of transportation in order to perform the major functions of the job.•Automobile liability insurance coverage for vehicle being used in accordance with Center requirements.•CPR training obtained within 90 days of employment and maintained annually.•Ability to work collaboratively as a team to coordinate care for youth and young adults with intensive needs.</p> <p>Pay Range:</p> <p>$19.58-$32.31</p> <p>EEO Statement:</p> <p>Brown University Health is an Equal Opportunity employer.
