You'll help identify physical, emotional, mental, and social needs while assisting with care coordination and discharge planning to ensure safe transitions of care.</p> <p>This role serves as a key liaison between patients, families, healthcare teams, and community resources, helping connect individuals with the support services they need both during hospitalization and after discharge.
