<p>Job Type: Regular</p> <p>Time Type: Full time</p> <p>Work Shift: Day (United States of America)</p> <p>FLSA Status: Exempt</p> <p>When you join the growing BILH team, you're not just taking a job, you're making a difference in people's lives.</p> <p>Job Description:</p> <p>What You'll Do:</p> <p>The Hospice Clinical Manager will oversee a hospice interdisciplinary team that is responsible for providing supportive care to Hospice patients.</p> <p>More Specifically:</p> <ul> <li>Oversee the management of patient care, coordinating all services and facilitating communication between all providers, including financial oversight; </li><li>Manage hospice staff members including, but not limited to: recruitment & hiring, training & support, and performance feedback. </li><li>Oversee patient assessments for program appropriateness and implementation of the referrals; </li><li>Facilitate and consult with health care givers, reimbursement agencies, mental health facilities, clergy, volunteer and other community resources; </li><li>Lead hospice conference with inter-disciplinary staff; </li><li>Educate partners in the Beth Israel Lahey Health system about the opportunities in Palliative and Hospice for home care patients. </li></ul> <p>What You'll Need:</p> <ul> <li>Registered Nurse from a Graduate of National League for Nursing accredited school of nursing </li><li>BSN strongly preferred </li><li>Current valid MA nursing license; </li><li>Current American Heart Association (AHA) CPR certification; </li><li>3 to 5 years of oncology or community health experience; </li><li>1-2 years' experience managing a team of clinicians in a home health setting; </li><li>Knowledge of Medicare/Medicaid regulations in hospice / homecare; </li><li>Strong interpersonal skills and the ability to facilitate cooperation and communication among team members. </li></ul> <p>What You'll Get:</p> <ul> <li>A highly competitive salary & benefits package, including generous PTO, 403(b), and tuition reimbursement; </li><li>A reasonable geographic territory with strong clinical support resources; </li><li>A highly inclusive diverse team that values the input of all staff to provide excellent patient care </li></ul> <p>As a health care organization, we have a responsibility to do everything in our power to care for and protect our patients, our colleagues and our communities.
