Health - RC5310 Job Description</p> <p>JOB DESCRIPTION - Rural Health Nurse Coordinator</p> <p>POSITION SUMMARY: The purpose of this position is to:</p> <p>1) implement grant goals, outcomes, and activities, including evaluation, that are related to chronic disease, including diabetes, hypertension, and cardiovascular health; and,</p> <p>2) to support training diabetes coalitions in the Appalachian Region as they implement community-based programs for diabetes and chronic-disease self-management. Under the direction of the Division of Community Health Director, the individual in this position will work as a member of a team of professionals on the chronic disease team to carry out the following duties:</p> <ul> <li>Assist in the planning, coordination, implementation, and evaluation of grant and program activities to meet outcomes. </li><li>Conduct data collection and analysis to measure program impact and effectiveness using mixed-methods quantitative and qualitative evaluation methods. </li><li>Attend conferences for training, networking, and professional development to stay up to date on the field of chronic disease, health promotion, and disease prevention. </li><li>Participate in the planning and implementation of community-based activities, meetings, conference, trainings, etc. </li><li>Develop clear and thorough evaluation summaries of findings to present in long-form documents and short-form presentations to best communicate program successes and challenges </li><li>Provide technical support to key stakeholders and partners to meet grant goals. </li><li>Use Microsoft Teams to virtually collaborate with teams and assist in the scheduling, agendas, and meeting minutes. </li><li>Efficiently and effectively coordinate communication among team members and with community partners and various stakeholders. </li><li>Lead training, implementation, technical assistance and evaluation of evidence-based chronic disease prevention and management programs (i.e., National DPP, DSMES, and CDSMP. </li><li>Assist with managing social media communications. Shares necessary information with co-workers in order that they may do the same. </li><li>Remains composed and takes actions to restore calm in stressful situations. </li><li>Cooperates with other departments/offices and work groups. </li><li>Communicates effectively with internal and external customers/partners. </li><li>Meets requirements for mandatory in-services and continuing education. </li></ul> <p>ORGANIZATIONAL SUPPORT RESPONSIBILITIES:</p> <ul> <li>Attendance - maintains good attendance in coordination with University policy. </li><li>Punctuality - reports to work on time and is ready for duty. </li><li>Uses time sheet to record time correctly for each pay period. </li></ul> <p>WORK ROLE RESPONSIBILITIES:</p> <ul> <li>Carries out written evaluation plan in accordance to meet all process and outcome indicators for the grant deliverables. </li><li>Disseminates data collection tools and conduct evaluation processes with program participants and community partners to evaluate the project </li><li>Participates in community meetings/events to increase project visibility, community awareness of the project, and partner participation </li><li>Schedules and organize meetings/events and maintains agenda to meet project goals </li><li>Maintains updated documentation and records of all meetings and events related to the project </li><li>Submits accurate and prompt data reports to grant PI </li><li>Assists PI in implementation, execution, and completion of grant goals, outcomes, and activities </li><li>Performs other duties as assigned or requested </li></ul> <p>OTHER REQUIREMENTS:</p> <ul> <li>Must have advanced knowledge and use of Microsoft Excel </li><li>Must be proficient in Microsoft Word, PowerPoint, and Outlook. </li><li>Must possess excellent time management and organizational skills, including the ability to prioritize quickly with the ability to multi-task </li><li>Must be detail oriented with results-driven approach </li><li>Must be skilled at developing rapport with clients and stakeholders from other organizations, and maintaining working relationships that support the implementation of services </li><li>Must have excellent verbal and written communication skills as well as demonstrated skills in public speaking, leadership, and group facilitation </li><li>Must demonstrate a clear understanding of professional boundaries and confidentiality when working with individuals and the professional staff in partner agencies </li><li>Must have a valid driver's license and reliable transportation </li></ul> <p>Location MUSOM - Marshall University School of Medicine Salary Range Salary is commensurate with education, experience and internal equity</p> <p>Required Qualifications</p> <p>Education</p> <ul> <li>Minimum Bachelor's degree in nursing, public health, or health-related field.
