<p>SUMMARY</p> <p>The Home Health Aide, Hospice assists terminally ill patients with their personal care requirements, maximizing their activities of daily living; and assists terminally ill patients in achieving the highest level of comfort and quality of life in their homes, consistent with their health status.</p> <p>Hours: On call</p> <p>Shift: 8:00am-4:30pm</p> <p>PRIMARY RESPONSIBILITIES</p> <ul> <li> <p>Assist terminally ill patients in achieving the highest level of comfort and quality of life in their homes, consistent with their health status and plan of care.</p> </li><li> <p>Perform or assist the patient in eating, dressing, taking of self-administered oral medications, preparing light meals, changing bed linens, cleaning and straightening patient's immediate personal environment and perform light housekeeping duties to facilitate patient's self care at home.</p> </li><li> <p>Report significant changes in patient's condition to Case Manager in a timely manner.</p> </li><li> <p>Create a successful interpersonal relationship with the patient and family and use encounters as an opportunity for psychosocial support, to observe cognitive functions and observe for environmental safety.</p> </li><li> <p>Provide hands-on personal care and assist with self-care skills in the home environment per the plan of care.</p> </li><li> <p>Assist with personal hygiene such as skin, mouth, hair care, bathing, foot soaks, make-up, shaving and fingernail care.</p> </li><li> <p>Perform personal hygiene tasks for patients who are unable to perform them without assistance, including total bed baths, turning and repositioning, perineal care, application of topical ointments (as directed) and oral/teeth care.</p> </li><li> <p>Assist patients to the bathroom/commode or in using bedpans/urinals.</p> </li><li> <p>Assist patients in performing activities related to mobility. Assist in and out of bed, with transfers, ambulation, passive range of motion and turning/repositioning.</p> </li><li> <p>Perform routine procedures/treatments as assigned by the Nurse.</p> </li><li> <p>Take vital signs: temperature, pulse, respirations, and blood pressure.</p> </li><li> <p>Perform simple non-sterile dressing changes.</p> </li><li> <p>Perform Foley/condom catheter cares, ostomy cares; changing/emptying ostomy collection bags.</p> </li><li> <p>Perform other procedures/treatments after direct instruction at the discretion of the Nurse.</p> </li><li> <p>Document all patient-care activities as assigned by Nurse according to departmental policies and procedures.</p> </li><li> <p>Participate in patient care coordination meetings.</p> </li><li> <p>Perform miscellaneous projects and complete various tasks as requested by management.</p> </li></ul> <p>JOB SPECIFICATIONS</p> <p>Education, Experience and Credentials</p> <ul> <li>High school graduate or equivalent. </li><li>Home Health Aide certification and/or competency testing required. </li><li>Six months experience as a Home Health Aide or Nursing Assistant preferred. </li><li>Possess a valid driver's license with good driving record, and proof of valid insurance. Must have reliable transportation. </li><li>CPR Certification required. </li></ul> <p>KNOWLEDGE, SKILLS, AND ABILITIES</p> <ul> <li>Able to uphold the rights of the individual hospice patients. </li><li>Able to plan, prioritize, coordinate, and manage own work. Able to work unsupervised, make decisions and solve routine problems independently, effectively and creatively. </li><li>Self-motivated, fast learner shows initiative, willing to seek out patient needs and meet them. </li><li>Understand and maintain confidential nature of organization and client information, including and in accordance to HIPAA regulations. </li><li>Work as an effective and proactive team-player; understand the importance of supporting the organization, patients, customers and other Sholom employees. </li><li>Able to display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization in accordance with our PROUD standards. </li><li>Computer skills, including Microsoft Word, Outlook, Internet, and other applications. </li></ul> <p>PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB</p> <p>Incumbent must be able to:</p> <ul> <li>Perform heavy work, lifting up to 50 pounds, and occasionally lifting/carrying of objects weighing up to 75 pounds. </li><li>Push/pull clients in wheelchairs up to 200 pounds a distance of several feet. </li><li>Vision acuity to read fine print, warning labels, doctor prescriptions, etc. </li><li>Communicate clearly and effectively both orally and in writing, including possessing good English speaking skills, fluency and understandability. </li><li>Must be able to successfully pass training and education modules, and exhibit competency with medication administration and documentation. </li><li>Twist, turn, bend, stoop, kneel, squat and reach frequently in the performance of client assistance responsibilities. </li><li>Frequently stand and walk during an eight-hour shift. </li><li>Perform repetitive movements of the upper extremities. </li><li>Exert moderate physical energy for up to eight hours per day. </li><li>Apply manual dexterity for using and maintaining tools and equipment. </li><li>Work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed. </li></ul> <p>TOOLS AND EQUIPMENT USED</p> <p>Incumbent must be able to use communication devices, medical equipment, patient assistive equipment, walkers, wheelchairs, electric wheelchairs, telephone, copy machines, computers, and medical equipment such as thermometers, blood pressure equipment, oxygen tanks, laundry equipment, cleaning and other equipment.</p> <p>WORK ENVIRONMENT</p> <p>The work is indoors in the residences of senior clients, but demanding. Moving heavy lifts, assistive equipment, cleaning equipment and patients are a consistent part of this job.</p> <p>This job description does not necessarily list all the functions or accountabilities of the job.
