Be the difference.</p> <p>Job Summary</p> <p>Provides physical therapy treatment under the direction of a licensed Physical Therapist according to the principles and practices of Physical Therapy. Collaborates with team members in the implementation of the patient's individualized plan of care. Provides patient/family education, completes documentation of patient treatments, charges and other information as needed. Added responsibilities for program and staff development is required. Be the difference</p> </li><li> <p>Implements and manages a specific and comprehensive treatment plan and participates in patient discharge planning:</p> </li><li> <p>Administers patient treatments within the plan of care (POC) outlined by a licensed physical therapist and progresses patients as indicated</p> </li><li> <p>Demonstrates basic knowledge of diagnoses/conditions seen in physical therapy</p> </li><li> <p>Interacts with others involved in the patient's care to incorporate an interdisciplinary treatment approach for coordinated quality care</p> </li><li> <p>Demonstrates awareness and use of proper safety measures regarding the patients, others in the home and self</p> </li><li> <p>Assesses patient response to treatment and modifies goals and treatments as indicated</p> </li><li> <p>Provides information regarding follow-up services which may include a referral to other community resources</p> </li><li> <p>Participates in patient and family education process.</p> </li><li> <p>Supports and provides educational material to patient/families for support groups as well as community resources</p> </li><li> <p>Provides education and training to patients, families, and caregivers regarding diagnoses, progress, and long-term functional outcomes</p> </li><li> <p>Teaches, counsels, and communicates with the patient, family or caregiver educational information related to the patient's diagnosis/condition</p> </li><li> <p>Complies with departmental policies for documentation.</p> </li><li> <p>Documentation is in compliance with chart review audit components</p> </li><li> <p>Documents all aspects of assessment, planning, treatment and discharge in the computer in a timely manner</p> </li><li> <p>Follows POC as defined by the primary therapist and documents the patient's progress toward goals</p> </li><li> <p>Daily treatment records are noted in the computer on a daily basis and are clear and professional in terminology.</p> </li><li> <p>Appropriate abbreviations and medical terminology are utilized</p> </li><li> <p>Obtains re-evaluations as required by S. C.
