<p>Job Overview:</p> <p>The PACE Activities Coordinator is a member of the PACE Interdisciplinary Team (IDT) and is responsible for completing recreational therapy assessments for new PACE participants and reassessments for existing PACE participants as part of the PACE IDT care planning process. The PACE Activities Coordinator is responsible for providing a comprehensive therapeutic recreation and activity program for PACE participants. Activities are provided primarily in the PACE Center on days of attendance but may also include group outings for participants in community locations.</p> <p>Job Requirements:</p> <p>Bachelor's Degree Recreational Therapy from an accredited college (Preferred)</p> <p>2 - 3 years Experience in a social or recreational program providing and coordinating recreation services for the frail or elderly population within the past 5 years, one of which was full-time inpatient activities program in a healthcare setting (Required)</p> <p>1 - 2 years Experience working with a frail or elderly population or, in the absence of experience, receive appropriate training from PACE on working with a frail or elderly population before providing direct patient care to PACE participants independently (Required)</p> <p>Experience Working in Electronic Records</p> <p>Reliable Transportation</p> <p>BLS BLS Preferred</p> <p>Job Responsibilities:</p> <p>Participates as a PACE Interdisciplinary Team (IDT) member and is responsible for completing comprehensive initial recreational therapy assessments for new PACE participants and reassessments on existing PACE participants as part of the PACE IDT care planning process.</p> <p>In collaboration with the IDT, develops and establishes realistic and measurable recreation therapy and activity goals for PACE participants. Develops and implements individual and group activities for PACE participants under the supervision of PACE Center management in a culturally and linguistically appropriate manner considering the medical, physical, social, and emotional needs of PACE participants</p> <p>Monitors recreation aides and other PACE staff at the direction of PACE Center management.</p> <p>Attends IDT and morning meetings and reports on participant status and engagement with PACE Center activities.</p> <p>Monitors PACE participants for changes in their health or psychosocial status requiring a recreational therapy reassessment.
