<p>Description</p> <p>Purpose: Serves as a liaison between the hospital, community and medical staff. Meets with patients to</p> <p>provide clinical counseling regarding bloodless care. Population served based on scope of services in the</p> <p>department.</p> <p>Essential Responsibilities</p> <ol> <li>Maintains relevant policies and procedures, patient education, outcomes data collection, quality </li></ol> <p>improvement efforts and education of relevant staff in the various aspects of the bloodless</p> <p>medicine for all patient population with a focus on patient safety.</p> <ol start="2"> <li>Supports the development of written protocols and education of nurses detailing the rapid </li></ol> <p>communication and basic response to critical vital signs that is specifically required to minimize</p> <p>delays in the diagnosis and treatment of serious adverse events. Serves as the clinical liaison</p> <p>between the facility and providers to ensure that all identified patients receive the required</p> <p>information as it relates to care.</p> <ol start="3"> <li>Responsible for the timely submission of outcomes data to the quality department and other </li></ol> <p>Committees as needed.</p> <ol start="4"> <li>Works closely with the multidisciplinary team to ensure positive patient outcomes throughout </li></ol> <p>the continuum of care for the bloodless medicine patients.</p> <ol start="5"> <li>Works with senior leadership to design communication strategies and relationship building to </li></ol> <p>support the strategic plans and revenue goals.</p> <ol start="6"> <li>Coordinates routine inpatient rounding with patients, family members, providers, inpatient </li></ol> <p>staff, nursing leadership, case management and physical therapy to educate on the blood</p> <p>management program.</p> <ol start="7"> <li>Contacts patients and family members pre, during and after discharge to ensure satisfaction </li></ol> <p>regarding their hospitalization. Conducts patient call back surveys within 72 hours of discharge.</p> <ol start="8"> <li> <p>Participates in survey readiness initiatives.</p> </li><li> <p>Participates and leads meetings including, Bloodless Care/Patient Blood Management</p> </li></ol> <p>Committee, QPIC and Service Line Specific committees as directed by the medical director and</p> <p>chief nursing officer.</p> <ol start="10"> <li>Collaborates across the health system with bloodless medicine on system initiatives including </li></ol> <p>system meetings and dashboards.</p> <ol start="11"> <li>Identifies and assesses needs of physicians and referral sources. Communicates and educates to </li></ol> <p>acquire new, or enhance existing, referral relationships</p> <ol start="12"> <li>Perform gap analysis and identify opportunity areas of local physician groups. Presents findings </li></ol> <p>and improvement programs.</p> <ol start="13"> <li> <p>Maintain and Model REACH Values (Respect, Excellence, Accountability, Compassion, Honor).</p> </li><li> <p>Demonstrates regular, reliable and predictable attendance.</p> </li></ol> <p>JOB DESCRIPTION</p> <p>Revised 7/2025 2</p> <ol start="15"> <li>Performs other duties as required. </li></ol> <p>Education and Experience Requirements:</p> <ul> <li>Graduate of an accredited school of nursing. </li><li>Three (3) years of progressive nursing experience in an acute care setting. </li><li>Minimum of one (1) year in a leadership role. </li><li>Minimum of Six (6) months of Clinical chart review and abstraction experience. </li><li>PREFER: Bachelor of Science in Nursing (BSN) </li><li>PREFER: Six (6) months experience with bloodless medicine patient population. </li></ul> <p>Minimum Knowledge, Skills and Abilities Requirements:</p> <ul> <li>Proficient in Microsoft Office software and Electronic Medical Record (EMR) systems. </li><li>Strong presentation skills. </li></ul> <p>License, Registration, or Certification Requirements:</p> <ul> <li>Current NYS Registered Nurse (RN) License </li><li>Basic Life Support (BLS) certified. </li></ul> <p>Environmental Factors:</p> <p>Factors affecting environment conditions may vary depending on the assigned work area and tasks.</p> <p>Potential environmental exposures include, but are not limited to:</p> <ul> <li>Bio-hazardous Waste </li><li>Blood Borne Pathogens.
