Primarily located at South Shore Hospital, but will cover community providers as needed.</p> <p>Essential Job Functions</p> <ul> <li>Identifies and prioritizes patients at community health care facilities who have requested home care services from SSVNA </li><li>Acts as consultant to SSH care managers/SSH staff regarding home care programs, community resources and alternative care settings </li><li>Establishes and develops relationships with colleagues, physicians, referral sources, patients and family members to facilitate the referral to admission process and acts as liaison between all parties </li><li>Explains the types of care and services the SSVNA offers to patients and their families </li><li>Recruits' referrals and coordinates with the Central Intake department in assisting the coordination of admissions while aligning with SSVNA's daily capacity </li><li>Assesses incoming patient's physical, social and psychosocial needs to ensure they meet the agency's admission criteria. </li><li>Consults with referral sources on complex patient discharges and coordinates patient's care with SSVNA </li><li>Obtains financial information, as necessary </li><li>Establishes open lines of communication with referral sources to promote good community relations </li><li>Works to provide excellence in service to existing accounts and strives to increase referrals </li><li>Participates in appropriate professional, community organizations, events and outreach activities </li><li>Reviews weekly Admission and Referral Reports and Dashboards. </li><li>Maintains statistical data of referrals and submits reports as required </li></ul> <p>Non-Essential Job Functions</p> <ul> <li>Non-essential functions are those tasks, duties and responsibilities that are not critical to the performance of the job.
