You'll play a role in something that's never been done before as we integrate science and clinical care to help patients achieve better, faster outcomes - as we Advance Human Ability, together.</p> <p>Job Description Summary</p> <p>The Admission Consultant will facilitate the referral and admission of patients from other sites of care to inpatient rehabilitation units at the Shirley Ryan AbilityLab (SRAlab) and SRAlab partner facilities, representing SRAlab to outside parties and ensuring medical appropriateness of transfers.</p> <p>The Admission Consultant will consistently demonstrate support of the Shirley Ryan AbilityLab statement of Vision, Mission and Core Values by striving for excellence, contributing to the team efforts and showing respect and compassion for patients and their families, fellow employees, and all others with whom there is contact at or in the interest of the institute.</p> <p>The Admission Consultant will demonstrate Shirley Ryan AbilityLab Core Attributes: Communication, Accountability, Flexibility/Adaptability, Judgment/Problem Solving, Customer Service and Core Values (Hope, Compassion, Discovery, Collaboration, and Commitment to Excellence) while fulfilling job duties.</p> <p>Job Description</p> <p>The Admission Consultant will:</p> <ul> <li> <p>Support the admission process to facilitate a smooth conversion from referral to admission by assessing the appropriateness for IRF, providing clinical information to SRAlab and communicating effectively with all parties.</p> </li><li> <p>Represent SRAlab rehabilitation services to various audiences within referral sources, primarily physicians and hospital discharge planners/case managers.</p> </li><li> <p>Review medical charts and communicate pertinent medical information to the internal medical team and external referrer when relevant.</p> </li><li> <p>Determine medical appropriateness of patients and support the admission process by gathering and transferring medical information to the appropriate inpatient admitting team.</p> </li><li> <p>Educate patients and families on the benefits of SRAlab and to facilitate the referral to admission.</p> </li><li> <p>Gather and use patient health information to assist as requested with treatment, payment, and healthcare operations.</p> </li><li> <p>Collect and analyze data and prepare reports related to customer service and throughput.</p> </li><li> <p>Monitor and report customer service issues to appropriate SRAlab personnel.</p> </li><li> <p>Manage the administrative aspects of customer service of a referral source by ensuring that databases reflect the referral/admission activities and maintain a current listing of all referral relations.</p> </li><li> <p>Partner with Admission Liaisons based in the field to coordinate the plan and admission when needed.</p> </li><li> <p>Perform all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLab.</p> </li></ul> <p>Reporting Relationships</p> <ul> <li>Reports directly to Director, Inpatient Access </li></ul> <p>Knowledge, Skills & Abilities Required</p> <ul> <li> <p>Bachelors Degree and Current Illinois license to practice as a registered nurse required.</p> </li><li> <p>Require a Masters Prepared Nurse Practitioner, Physician Assistant or Registered Nurse</p> </li><li> <p>A minimum of five years of clinical nursing experience required; rehabilitation experience and CRRN strongly preferred.</p> </li><li> <p>Demonstrated ability to communicate clearly and knowledgeably with patients, families and clinicians to discuss treatment options.</p> </li><li> <p>Exceptional communication skills, both verbal and written.</p> </li><li> <p>Understands excellent customer service and has successful proven experience in applying customer service skills.</p> </li><li> <p>Technically competent in using all electronic communications including a lap top computer, pagers and cell phones.</p> </li><li> <p>Highly organized, detail oriented, while understanding the 'big picture', and takes ownership to successfully meeting strict deadlines.</p> </li></ul> <p>Working Conditions</p> <ul> <li>Normal office environment with little or no exposure to dust or extreme temperature. </li></ul> <p>The above statements are intended to describe the general nature and level of work being performed by people assigned to this classification.
