<p>Employment Type:</p> <p>Part time</p> <p>Shift:</p> <p>Description:</p> <p>Position: Speech Therapist</p> <p>Location: Enfield/Stafford Area</p> <p>Employment Type: Part-Time, 24 hours weekly</p> <p>Benefits Start: Day 1</p> <p>Sign on Bonus: $2500</p> <p>Position Purpose:</p> <p>As a Speech Therapist at Trinity Health Of New England At Home, you will provide professional speech and audiology diagnostic and therapeutic services to agency clients as authorized by their physician.</p> <p>Key Responsibilities:</p> <ul> <li>Client Assessment & Planning: </li></ul> <p>Accurately assess clients at admission and required time points using standardized documentation. Develop a treatment plan in collaboration with the physician and client, focusing on achievable goals to improve or stabilize the client's condition.</p> <ul> <li>Visit Prioritization: </li></ul> <p>Effectively prioritize and plan visits based on client capabilities, condition, physician orders, and available resources.</p> <ul> <li>Regulatory Compliance: </li></ul> <p>Maintain a working knowledge of Federal, State, and local laws and regulations, including the Organizational Integrity Program, Code of Ethics, and agency policies. Adhere to these standards with honesty, ethics, and professionalism.</p> <ul> <li>Care Coordination: </li></ul> <p>Make appropriate referrals to other disciplines and services. Coordinate care to ensure effective and efficient service delivery.</p> <ul> <li>Intervention & Education: </li></ul> <p>Implement preventive, therapeutic, and rehabilitative procedures according to the treatment plan.
