Full Time Shift Day Work Schedule Summary</p> <p>Monday - Friday, Hybrid with flexible working hours between 8am - 5pm.</p> <p>VP Area U of U Health - Academics Department 00267 - Ophthalmology-Services Location Campus City Salt Lake City, UT Type of Recruitment External Posting Pay Rate Range $56,000-60,000 Close Date 07/16/2026 Priority Review Date (Note - Posting may close at any time) Job Summary</p> <p>Community Outreach Specialists</p> <p>The UTOP Tele-Retina Program is a telehealth initiative focused on increasing access to diabetic retinopathy screening and eye care services in undeserved communities. The Program Coordinator will play a critical role in expanding the program's reach by building relationships with local healthcare providers and coordinating follow-up care for patients identified with retinal disease.</p> <p>Learn more about the great benefits of working for University of Utah: benefits.utah.edu</p> <p>The department may choose to hire at any of the below job levels and associated pay rates based on their business need and budget.</p> <p>Responsibilities</p> <p>Provider Engagement & Outreach</p> <ul> <li>Identify and reach out to local primary care clinics, optometrists, and health systems to participate in the tele-retina screening network. </li><li>Develop and maintain relationships with community providers and clinic leadership to ensure sustained participation and collaboration. </li><li>Conduct informational sessions and presentations to educate clinics and providers about the benefits of the UTOP Tele-Retina Program. </li></ul> <p>Care Coordination & Patient Follow-Up</p> <ul> <li>Collaborate with Federally Qualified Health Centers (FQHCs) to establish referral pathways and ensure timely treatment for patients with identified retinal conditions. </li><li>Coordinate appointments for specialty care and assist with navigation to appropriate follow-up services. </li><li>Track patient outcomes and communicate updates to referring providers and care teams. </li></ul> <p>Program Administration</p> <ul> <li>Maintain accurate and up-to-date records of outreach activities, clinic participation, and patient referrals. </li><li>Assist with scheduling, data entry, and communication workflows between screening sites and the retinal specialists reviewing the images. </li><li>Support quality improvement initiatives and data collection to evaluate program effectiveness. </li></ul> <p>Collaboration & Communication</p> <ul> <li>Work closely with the UTOP clinical and outreach teams to align program goals and share feedback from community partners. </li><li>Facilitate meetings, training sessions, and ongoing communication with participating providers and clinic staff. </li></ul> <p>Minimum Qualifications</p> <p>EQUIVALENCY STATEMENT: 1 year of higher education can be substituted for 1 year of directly related work experience (Example: bachelor's degree = 4 years of directly related work experience).</p> <p>Department may hire employee at one of the following job levels:</p> <p>Community Outreach Specialists, III: Requires a bachelor's (or equivalency) + 6 years or a master's (or equivalency) + 4 years of directly related work experience.</p> <p>Community Outreach Specialists, IV: Requires a bachelor's (or equivalency) + 8 years or a master's (or equivalency) + 6 years of directly related work experience.</p> <p>Preferences</p> <ul> <li>Strong interpersonal and communication skills, with experience in community outreach or provider engagement. </li><li>Knowledge of healthcare systems, care coordination, or telehealth services preferred. </li></ul> <p>Type Benefited Staff Special Instructions Summary Additional Information</p> <p>The University is a participating employer with Utah Retirement Systems ("URS").
