Your recruiter will provide the most up-to-date details during the hiring process.</p> <p>How you'll make an impact in this role</p> <p>Core Pharmacist Responsibilities: Safe and Effective Medication Management</p> <p>Order Management: Review and validate all medication orders for clinical appropriateness. Screen for interactions, duplications, and contraindications. Resolve issues with prescribers and document interventions.</p> <p>Preparation & Dispensing: Accurately compound specialized sterile/non-sterile medications (e.g., chemo, TPN) per regulatory standards. Ensure correct dispensing, labeling, and inventory control.</p> <p>Clinical Expertise & Education: Provide expert pharmacotherapy information to providers. Offer comprehensive, patient-centered counseling on complex medications (action, administration, side effects, adherence).</p> <p>Collaboration & Care Coordination: Participate in rounds and committees. Develop patient-specific medication plans. Perform medication reconciliation upon admission/discharge to prevent errors and ensure safe transitions of care.</p> <p>Quality & Development: Supervise technicians, ensuring quality control and regulatory compliance. Mentor students/residents.
