<p>Empath Home Health, a member of Empath Health, is seeking a PRN Lymphedema Certified OT - Occupational Therapist. The Home Health Lymphedema Certified OT Occupational Therapist provides occupational therapy services related to patient's level of function and performance of daily living.</p> <p>Locations: home health visits available in Manatee county</p> <p>Schedule: PRN</p> <p>The Home Health Lymphedema Certified OT - Occupational Therapist:</p> <ul> <li>Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations. </li><li>Performs initial and on going assessments to determine level of functioning and patient's ability to perform activities of daily living, including OASIS assessments at appropriate time points. </li><li>Develops and revises a plan of care in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of self-care. </li></ul> <p>Requirements:</p> <ul> <li>Bachelor Degree in Occupational Therapy. </li><li>Florida licensure in occupational therapy. </li><li>Minimum of one (1) year experience as a licensed Occupational Therapist. </li><li>CLT - Certified Lymphedema Therapist in the respective state. </li><li>Home health care experience preferred. </li></ul> <p>Suncoast Hospice, Empath Home Health, and Empath Health Pharmacy are proud to be accredited by the Joint Commission showing our commitment to quality.</p> <p>Empath Health values diversity as it strengthens our community and care.
