<p>Join the Imagine! team with this new position! The focus is to support leadership with the ongoing operations of Imagine! Residential Wrap-around Services. Works with individuals, families, and teams to access available internal and external resources that promote well-being. Ensure program quality, maintains regulatory and licensure compliance, and contributes to the financial viability of the program. This position is responsible for ensuring the health and safety of the participants and maintaining professional relationships with Imagine! participants and stakeholders.</p> <p>Essential Duties/Responsibilities</p> <p>Assists in ensuring program quality of Imagine! Residential Wrap-around Services.</p> <ul> <li>Plans, coordinates, and provides program activities, including scheduling of staff, vehicles, and transportation, ensuring the efficiency and quality of programming. </li><li>Maintains a caseload of clients, including a schedule of billable activities and supports. </li><li>Leads continuous improvement initiatives aimed at enhancing service delivery, efficiency, and client satisfaction. </li><li>Maintains regular contact with team members, individuals, and families to provide updates, address concerns, and support service delivery. This includes, but is not limited to, monthly check-ins and quarterly meetings. </li><li>Coordinates wrap-around services to ensure individuals meet personalized goals, ensuring alignment with Imagine!'s person-centered practices. </li><li>Meets regularly with service leadership and attends staff meetings to stay informed of individual progress and provide individual specific training. </li></ul> <p>Promotes the health, safety, and welfare of the participants.</p> <ul> <li>Ensures health and safety requirements set forth by Imagine! guidelines as well as all regulating agencies. </li><li>Supports individuals with skill building to accomplish their identified goals. </li><li>Provides direct support by assisting with navigation of services, facilitating activities, and addressing immediate needs. </li><li>Assists individuals in accessing and utilizing community-based services and service providers through coaching and mentorship. </li></ul> <p>Assists the Program Manager in maintaining regulatory and licensure compliance of the program.</p> <ul> <li>Assists the Program Manager with program compliance with all standards and regulations, including Imagine! policies and procedures. </li><li>Monitors and tracks data to ensure programmatic compliance and the overall effectiveness and performance of the program. </li><li>Maintains up-to-date knowledge of Imagine! services, regulatory requirements and industry best practices. </li></ul> <p>Contributes to the financial viability and integrity of the program.</p> <ul> <li>Creates and maintains program schedules, registration, and attendance. </li><li>Provides services approved in the Person Centered Support Plan (PCSP) to clients served on a variety of HCBS waivers </li><li>Practices fiscal responsibility through budget control as established in company policy and procedure. </li><li>Participates in organizational and departmental efforts to improve processes and procedures. </li></ul> <p>Other Duties/Responsibilities</p> <ul> <li>Maintains and cares for Imagine! property, including supplies, vehicles, etc., being mindful of safety and cleanliness. </li><li>Represents program in meetings and committees, as well as in promoting and marketing, as requested. </li><li>Maintains professional relationships with Imagine! stakeholders. </li><li>Performs other duties as assigned. </li></ul> <p>This position requires regular use of a personal vehicle with liability insurance that meets Imagine! standards.</p> <p>Job Qualifications</p> <p>Knowledge, Skill, and Ability:</p> <ul> <li>Demonstrated skill in working with people with developmental disabilities, preferably in a community-based setting. </li><li>Effective written and verbal communication skills with individuals and groups at all professional levels. </li><li>Effective and creative negotiator and problem solver. </li><li>Ability to work independently and prioritize tasks for self and others. </li><li>Ability to work effectively with various technology including word processing software, internet based databases, assistive technology, etc. </li><li>Exceptional organization and collaboration skills. </li><li>Effective trainer. </li><li>Ability to analyze data, identify trends and make informed recommendations. </li><li>Possession of a valid driver's license and ability to meet Imagine! driving requirements. </li><li>This position requires regular use of a personal mobile device such as a smartphone or tablet. </li></ul> <p>Training/Education:</p> <ul> <li>High School diploma required. </li><li>Bachelor's degree in human services preferred. </li><li>Satisfactory completion Imagine! and job-specific training. </li><li>ACRE Basic Employment Services Certificate or obtain within 90 days after being hired </li></ul> <p>Experience:</p> <ul> <li>One year of experience working in the field of intellectual and developmental disabilities required. </li><li>Experience with documentation and compliance systems, such as an EHR, preferred. </li></ul> <p>Working Environment/Physical Activities</p> <ul> <li>Ability to lift 50. </li><li>Ability to work occasional weekends and evenings. </li><li>Ability to drive a van with lift. </li><li>Ability to push and pull participants using wheelchairs. </li><li>Ability to physically lift, transfer, and assist participants as related to toileting, personal needs, and activities. </li><li>Activities take place in the community. </li><li>Effective with shifting roles, responsibilities, and expectations in a fast paced and changing environment. </li><li>Ability to respond to emergency situations. </li></ul> <p>Note: This job description is not intended to be an exhaustive list of all duties, responsibilities, or qualifications associated with the job.</p>
