We are driven by Positivity which inspires us to be our best and to go above and beyond for our patients, for one another, and for our community.</p> <p>If you're ready for a rewarding new career, join Team Bronson and be part of the experience.</p> <p>Location</p> <p>BMH Bronson Methodist Hospital</p> <p>Title</p> <p>Social Service Liaison - Full Time 80Hrs/Pp - Bronson Methodist Hospital</p> <p>Reporting to the System Manager of Case Management the Social Services Liaison (SSL) manages and assists in the flow of patient access, discharge and follow up functions for patients with a high utilization rate in the BMH Emergency Department. The SSL provides case management for patients with complex psychosocial needs and facilitates solutions to the resource needs, behavioral health needs, and psychosocial needs. This positions coordinates, negotiates, procures, and facilitates the utilization of resources for patients to provide high-quality, cost-effective outcomes to reduce hospital utilization and readmissions. The SSL will develop collaborative relationships with community partners and patients developing strategies connecting patients with resources to improve their overall health.</p> <p>Responsibilities:</p> <ul> <li>Work with multidisciplinary team and providers to create standard care plan on patients. Review care plan and/or behavioral contract with patient as needed. </li><li>Reassess and modify care plans as needed. </li><li>Provide appropriate interventions which demonstrate knowledge of and sensitive to different cultural backgrounds, health literacy, and educational backgrounds of the population served. Utilize interpreter services per policy. </li><li>Perform psychosocial assessment including barriers to coping, mental health history, cognitive development, adverse developmental events, resistance to treatment, and mental health symptoms. </li><li>Assess eligibility of uninsured and underinsured patients for federal, state and community programs to assist funding of medical care. Assist patient with processing paperwork and collaborate with Patient Financial Counselors as needed. </li><li>Work with ED Medical Social Worker to address depression and or suicidal ideation and determine crisis response/action to take for resolution. </li><li>Educate patients on advanced directive and advanced care planning and assist with the completion of documents. Address end of life issues in collaboration with others. </li><li>Function as a liaison to community programs, participate in community meetings and maintain a network of appropriate contacts to identify potential resources for meeting patient's needs. </li><li>Promote empowerment of patient in self-management of disease. </li><li>Participate in meetings and organizational groups as requested. </li><li>Assess patient SDOH needs and provide resources as needed. </li><li>Make referrals to appropriate level of care and assist with arranging services per the needs of the patient. </li><li>Assure implementation of care plans throughout the continuum of care to include post-acute providers. </li><li>Utilize motivational interviewing techniques to support goals and reduce barriers to achieving them. </li><li>Track and analyze data as needed. </li></ul> <p>Job Requirements:</p> <p>Education: BSW, Counseling, or Behavioral Science related degree</p> <p>Work Experience: (2) years case management experience in a behavioral health setting</p> <p>Required Skills:</p> <ul> <li>Excellent organizational and problem-solving skills, ability to be flexible, and creative. </li><li>Knowledge of and ability to work within a complex healthcare system including applied practice and aptitude in advocating for patient needs while balancing organizational needs. </li><li>Demonstrate ability to work collaboratively as a member of the interdisciplinary team. </li><li>Superior verbal and written communication, behavioral interviewing skills, assessment skills including psychosocial assessment, and the ability to work with a varied population. </li><li>Proficient in Microsoft office, Power Point, and excel. </li><li>Excellent customer service skills and patient/family centered care. </li><li>Ability to work independently and to see consultation as necessary. </li><li>Ability to work in a high volume fast/paced environment. </li></ul> <p>Shift</p> <p>First Shift</p> <p>Time Type</p> <p>Full time</p> <p>Scheduled Weekly Hours</p> <p>40</p> <p>Cost Center</p> <p>2450 Case Management/Medical Social Work (BMH)</p> <p>Agency Use Policy and Agency Submittal Disclaimer</p> <p>Bronson Healthcare Group and its affiliates ("Bronson") strictly prohibit the acceptance of unsolicited resumes from individual recruiters or third-party recruiting agencies ("Recruiters") in response to job postings or word of mouth.
