<p>POSITION SUMMARY</p> <p>The Community Health Worker engages the community in the services and activities of the Health & Wellness Center and provides education, outreach and support to the community as well as individuals and families served. The Community Health Worker works within the multi-disciplinary team to reduce disparities to care, supports individuals and families with specific disease management, increase access to primary care, behavioral health and community resources. Wheeler Health & Wellness provides an integrated, comprehensive continuum of patient-centered primary care, behavioral health and community resources to enhance health equity and achieve the triple aim of health reform: improve health, reduce costs & increase quality.</p> <p>Rate: $20</p> <p>EDUCATION AND EXPERIENCE/QUALIFICATIONS</p> <p>High School diploma/GED and a minimum of two years of experience working with individuals and families in communities is required. Bachelors degree and some experience preferred.</p> <p>Knowledge of or residence in the target community of Hartford, strongly preferred.</p> <p>Valid Drivers License is required. Safely operates vehicle in the community, based on program needs.</p> <p>Ability to work on a team to ensure program goals are met, communicate effectively verbally and in writing, and is committed to personal and program growth. Ability to manage time effectively. Here's how we invest in you:</p> <p>Nurture Your Health:</p> <ul> <li>Access to wellness programs to support your physical and mental health </li></ul> <p>Fuel Your Career Growth:</p> <ul> <li>Access training and development opportunities </li></ul> <p>Maintain Work-Life Harmony:</p> <ul> <li>Access free and confidential counseling through our Employee Assistance Program (EAP) </li></ul> <p>ESSENTIAL DUTIES AND RESPONSIBILITIES</p> <p>Provides outreach to traditional and non-traditional settings in the cities and towns covered by the Greater Hartford Coordinated Access Network (CAN) to assist in the identification of individuals who are homeless who need connection to housing supports, medical services, and behavioral health services, and assistance with basic needs.</p> <p>Makes contact with individuals who are homeless within the community, at emergency departments and psychiatric units, and SNAP sites to engage in medical and behavioral health services.</p> <p>Makes formal treatment referrals for and helps to engage adults in treatment services and recovery supports.</p> <p>Collaborates with other providers within the region and develops linkages with organizations that provide services to individuals who are homeless.</p> <p>Maintains records of program activities for reporting and tracking purposes.</p> <p>Attends program staff meetings and assigned community meetings.</p> <p>Responsible for safely driving and operating the Mobile Medical Unit, including all associated operations and maintenance tasks. Tasks may include, but are not limited to operating and/or maintaining the unit's slide, leveling system, and gas.</p> <p>Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.</p>
