<p>Description</p> <p>Be a part of the mission to strengthen our communities by improving their health. The Hennepin County Mental Health Center is looking for a skilled Licensed Practical Nurse to support Hennepin County residents to achieve their health goals and recovery through access to low-barrier and high-quality health interventions provided at two Minneapolis clinic locations.</p> <p>Current Hennepin County employees who refer a candidate hired into an open competitive position may be eligible for a $500 referral bonus. Work hours vary depending on location and start as early as 8 a.m. and end as late as 6 p.m., Monday through Friday. Some occasional evening availability may be required. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. Details about NEO will be provided upon hire.</p> <p>About the position type:</p> <p>This is a full-time, benefit earning position.</p> <p>This position is a Nurse, Licensed Practical.</p> <p>Click here to view the job classification specification.</p> <p>In this position, you will</p> <ul> <li>Provide practical nursing care to patients and their families. </li><li>Collect and validate physical and psychosocial data, including vital signs. </li><li>Contribute to the initial and ongoing assessment of patient care needs. </li><li>Contribute to the development of patient care, teaching and discharge plans. </li><li>Observe and monitor patients. </li><li>Report deviations from normal, response to treatment, and changes in behavior. </li><li>Communicate with members of the health care team. </li><li>Administer prescribed medications, including the management of medication refills. </li><li>Administer prescribed treatments, such as basic wound care. </li><li>Assist in physical examinations. </li><li>Assist physician in performing treatments as needed, including the management of IV therapy. </li><li>Assist patients in activities of daily living. </li><li>Assist in emergencies, such as cardiac arrest. </li><li>Provide teaching to meet needs and families of patients according to teaching plan. </li><li>Review written discharge instructions with patients and families. </li><li>Document on appropriate forms and complete discharge forms. </li><li>Perform care coordination, including set up appointments and transportation to medical facilities and appointments outside of Hennepin County. </li><li>Monitor performance of Licensed Practical Nurse, Nursing Assistant, Health Unit Coordinator, and Health Care Aise as assigned. </li><li>Work as the nurse in charge when assigned by a supervisor under general supervision. </li><li>Participate in quality improvement activities. </li></ul> <p>Need to have:</p> <ul> <li>Completion of an approved program in practical nursing and successful completion of the State of Minnesota's (or equivalent) nursing exam. </li><li>Possession of a license as a Licensed Practical Nurse (LPN) authorized by the State of Minnesota. </li></ul> <p>Nice to have:</p> <ul> <li> <p>One year or more experience providing professional nursing in behavioral health or integrated care setting.</p> </li><li> <p>Current CPR certification.</p> </li><li> <p>Experience:</p> </li><li> <p>Administering medications via intramuscular route.</p> </li><li> <p>Working with laboratory management, point-of-care testing, or phlebotomy.</p> </li><li> <p>Using electronic health records to schedule and register clients and document client data/care.</p> </li><li> <p>Working with:</p> </li><li> <p>Underserved populations.</p> </li><li> <p>Adults experiencing homelessness and mental illness or co-occurring psychiatric and substance use disorders.</p> </li><li> <p>Or in a primary care clinic, home care, or performing care coordination duties.</p> </li><li> <p>Knowledge of:</p> </li><li> <p>Mental health diagnosis and medications</p> </li><li> <p>Knowledge of The Substance Abuse and Mental Health Services (SAMHSA) 10 x 10 Wellness Campaign</p> </li></ul> <p>Candidates with fluency in another language in addition to English are encouraged to apply.
