We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education and early childhood education programs to Medi-Cal eligible individuals and families throughout Alameda, Contra Costa, Fresno, Kings, Los Angeles, Madera, Orange, Placer, Riverside, Sacramento, San Bernardino, San Francisco, Santa Clara, Solano, Stanislaus, Stockton, Tulare and Ventura Counties.</p> <p>Program Type: FSP, MHSA, OP</p> <p>Population Served: 18 and up</p> <p>POSITION SUMMARY:</p> <p>The Mental Health Housing Coordinator works in direct collaboration with the Housing Department to coordinate housing services for consumers enrolled in the non-Full Service Partnership (FSP) programs. Works as part of a multi-disciplinary team to provide housing and mental health services to those experiencing homelessness and/or imminent risk or homelessness. Responsibilities include but are not limited to: outreach and screening potential candidates for Section 8 / Shelter Plus Care; overseeing/conducting the application process including tracking of outcomes; ensuring the appropriate match of supportive services; conducting housing search, placement and follow-up with mental health team.</p> <p>ESSENTIAL DUTIES AND RESPONSIBILITIES:</p> <ul> <li>Serves as the primary point of contact between the mental health non-FSP programs and the Housing Department. </li><li>Coordinates and provides support to mental health workers who are providing housing services. </li><li>Provides individualized supportive services to assess and address each client's needs and goals; including collaborating with mental health team to overcome individual barriers related to housing. </li><li>Provides rehabilitative and case management services to clients which might include assistance in restoring or maintaining a client's functional skills, daily living skills, and linkage to needed community services and support resources. </li><li>Completes Client Treatment Plans, progress notes, and other DMH required documentation within two to three business days and meets the standards of the Department of Mental Health, HUD, Pacific Clinics and its funding sources. </li><li>Assists clients with locating and securing permanent housing; conducts housing search and relocation services. </li><li>Screens referrals/applicants for Shelter Plus Care and Homeless Section 8 program eligibility requirements, including certification of homelessness and verification of disabilities. </li><li>Assists with completion and submission of applications for housing (both subsidized and unsubsidized), including assisting participants in obtaining necessary documentation. </li><li>Establishes professional working relationships with landlords, property management companies, and community agencies in an effort to build housing stock. </li><li>Identifies and develops a database of affordable housing resources. </li><li>Drives Agency-provided or personal vehicles for consumers, staff and others, as needed </li><li>Collects program data to assist with the completion and submission of HUD annual progress reports. </li><li>Maintains compliance according to HUD regulations, Clinics' guidelines and program requirements. </li><li>Conducts unit pre-inspections, quarterly site visits and annual inspections in accordance and compliance with HUD regulations. </li><li>Provides landlord/tenant mediation services. i.e. eviction prevention, mutual termination agreements, conflict resolution. </li><li>Completes Welligent / HMIS Clarity data entry and charting/documentation expectations within the timelines established by the program for service delivery per Agency requirements and provides services consistent with program needs pursuant to funding source directives, Pacific Clinics' protocol and HIPAA compliance requirements. </li><li>Completes and submits client service match, quarterly progress reports, status change and housing reports as required. </li><li>Assists the Pacific Clinics Housing Department to ensure compliance with HUD regulations and Clinics' guidelines. </li><li>Attends and participates in staff meetings to provide input towards consumer care, program development and staff training. </li><li>Reports to work on time and maintains reliable and regular attendance. </li><li>Models Pacific Clinics' approach, mission and core values in all communication and correspondence. </li><li>Communicates effectively in a culturally competent and diverse consumer population and promotes favorable interaction with managers, co-workers and others. </li><li>Performs other duties as assigned. </li></ul> <p>QUALIFICATIONS:</p> <p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.</p> <p>ORGANIZATIONAL RELATIONSHIPS/INTERACTION:</p> <p>Initiate and maintain professional interactions and communication with Clinic's employees and/or others.</p> <p>EDUCATION and/or EXPERIENCE/POSITION REQUIREMENTS:</p> <ul> <li>A Bachelor's degree in a Social Work or related field, or three (3) years of related experience with case management services, homeless and mentally ill populations, highly preferred. </li><li>Experience providing housing services and knowledge of federal and local housing programs preferred. </li><li>Must possess strong computer skills; including a working knowledge of Microsoft Office (Word, PowerPoint, Excel) and Microsoft Outlook. </li><li>Effectively coordinate housing activities in multiple service sites both within the agency and in the community. </li><li>Demonstrated experience and ability to relate to a culturally diverse clientele and staff. </li><li>A bilingual skill in the appropriate language for the Clinics' clients is highly preferred. </li><li>Must be able to work under minimal supervision. </li><li>Possess Strong community outreach skills. </li><li>Communicate effectively both verbally and in writing. </li><li>Strong paperwork and documentation skills. </li><li>Must possess a valid California driver's license and maintain an insurable driving record under the Clinics' liability policy. </li></ul> <p>PHYSICAL DEMANDS:</p> <p>While performing the duties of this job the employee is frequently required to stand or sit. The employee is required to use hands to produce records and/or documentation in manual or electronic format. The employee must the ability to ensure significant communication with the Clinics' community members. The employee must regularly lift and/or move up to 5 pounds and occasionally move or lift up to 10 pounds.</p> <p>The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.</p> <p>WORK ENVIRONMENT:</p> <p>The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.</p> <p>While performing the duties of this job, the employee is occasionally exposed to moving mechanical parts and outside weather conditions. The noise level in the work environment is usually moderate.
