Provides information about home care services, eligibility criteria, and policies to patients and families. Utilizes health record management and other software to process the flow of documentation required to schedule and deliver services to patients. Responds to questions and resolves issues to ensure customer satisfaction.</p> <p>Schedule: Monday- Friday 7:30am-4pm, 30 minute lunch| Location: Brier, San Bernardino, CA</p> <p>ESSENTIAL FUNCTIONS AND DELIVERABLES</p> <ul> <li>Engage and enroll new patients into the program through in-person or telephonic outreach. Work with team members to ensure team caseloads meet programmatic expectations. Gain a thorough understanding of program eligibility and expectations. Assist patients in maintaining eligibility to participate in the program. </li><li>Actively manage the eligible patient population working with internal and external stakeholders. </li><li>Monitor and analyze data exchange between SAC and external payors to ensure accurate data transfers. Collaborate with internal and external stakeholders to resolve technical issues and escalate them as necessary. </li><li>Complete enrollment assessments as required by the program and oversee patient benefit approval from start to finish. </li><li>Maintains working knowledge of specialized case and utilization management methodologies and practices; applies concepts to everyday practice. Maintain a working knowledge of insurance and community-based resources to aid in appropriate continuity of care. </li><li>Manage team caseloads ensuring patients are actively engaged. Disengaged patients will be dis-enrolled from the program and newly enrolled patients will be transitioned into team caseload. Utilize measurement-based and data-driven care methodologies to strategize and prioritize team caseloads. </li><li>Oversee patient eligibility for the program and track populations of focus. Provide feedback to leadership on eligible patient population and team needs to serve the patient population. </li><li>Manage clinic referrals and work collaboratively with Physicians and the referrals department to either enroll patients or link patients to services. </li><li>The position will assist teams with case management and projects as needed. </li><li>Ability to manage multiple assignments effectively; compose written material; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail. </li><li>Must be able to work cohesively within a team environment. </li><li>Home and/or community-based visits may be required based on patient and team needs. This position will travel with mobile clinics as needed to serve populations of focus. </li><li>Ability to collaborate, teach, support, and function in a teaching organization that builds coaches, and embraces interns and residency programs. Must demonstrate a passion for the team and can consistently work with rotating doctors, residents, and interns, while continuously building a trusting, safe, patient-centered workplace. </li><li>Maintain working knowledge of program requirements and stay up to date on internal and external expectations. Consistently work towards meeting program and organizational expectations including value-based care requirements. Actively participate in program and organizational training. </li><li>Ability to be flexible to assist in all departmental areas when needed and travel to other clinics as necessary; must have a reliable vehicle, valid driver's license, and auto insurance. </li><li>Other duties assigned. </li></ul> <p>QUALIFICATIONS:</p> <ul> <li>Education: A High School diploma or equivalent is required. An Associate's degree is preferred. </li><li>Licensure/Certification: Current CPR/BLS certification (must be American Heart Association or Red Cross accredited program), valid California driver's license, and auto insurance. Medical Assistant Certification or Community Health Worker Certification is also Required. </li><li>Experience: 3+ years working within a community clinic or related experience. 1+ year working within Quality programs or Care Management required. Prior experience working with mental health, substance use, or homeless populations is highly preferred. Bilingual in English and Spanish preferred. </li><li>Essential Technical/Motor Skills: Must be proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook). Must be able to use widely support internet browsers. Must have the ability to use variations of electronic health records and other various databases. </li><li>Interpersonal Skills: Must have excellent communications skills both orally and in writing. Must possess the ability to communicate with and relate to a diverse group of people including patients, community, and other staff. Must have strong conflict and problem resolutions skills. Must demonstrate the ability to supervise professional and para-profession staff while handling multiple tasks. </li><li>Essential Mental Abilities: Must be flexible to perform a variety of tasks. Must be well organized and a self-starter. Must have strong analytical and problem-solving skills. </li><li>Work Eligibility: Must be legally authorized to work in the United States on a full-time basis.
