<p>The Community Health Coordinator (CHC) is responsible for helping members and their</p> <p>families to navigate and access community services, and other resources, to support</p> <p>Social Drivers of Health including overall and maternal health, housing, transportation and</p> <p>early childhood education. The CHC may provide person-centered support that positively</p> <p>impacts healthy behaviors. The CHC coordinates referrals to providers and the Case</p> <p>Managers to support an integrated approach to access care and social support services.</p> <p>As a priority, activities will promote, maintain, and improve the health of members and their</p> <p>family. CHC provides social support and informal counseling, advocates for individuals and</p> <p>community health needs.</p> <p>FLSA Status</p> <p>Non- Exempt</p> <p>Salary Range</p> <p>$23.00/hr -$27.00/hr</p> <p>Reports To</p> <p>Manager</p> <p>Direct Reports</p> <p>None</p> <p>Location</p> <p>Los Angeles County - Placement will be within one of the listed center locations: · El Monte</p> <ul> <li>South Los Angeles · East Los Angeles · West Los Angeles · Panorama City · Lynwood </li><li>Lincoln Heights · Inglewood </li></ul> <p>Travel</p> <p>Up to 100%</p> <p>Work Type</p> <p>Regular</p> <p>Schedule</p> <p>Part- Time</p> <p>CRCLanguages El MonteEnglish, Spanish, Chinese, Vietnamese, Cantonese, Lincoln Heights English/SpanishInglewood English/SpanishLynwood English/SpanishSoLAEnglish/SpanishPanorama CityEnglish/SpanishWest LAEnglish (Spanish a plus)East LA English/ Spanish</p> <p>Position Description</p> <ul> <li>Educate members about healthcare management services, connecting them with </li></ul> <p>enrollment and serving as a liaison between the member/family and any services they</p> <p>may need.</p> <ul> <li>Lead new member orientations. </li><li>Support individuals and family as they navigate the health care system, and assist </li></ul> <p>members in accessing health-related services and community resources, such as</p> <p>transition to improvement in self-care, health care management, and health literacy.</p> <ul> <li>Establish trusting relationships with members and their families while providing general </li></ul> <p>support and encouragement.</p> <ul> <li>Provide ongoing follow-up and goal setting with members/families. </li><li>Communicate effectively and work collaboratively with all parties (members, families, </li></ul> <p>colleagues, and community-based organizations) as needed.</p> <ul> <li>Remove barriers that prevent members from accessing health and social services, such </li></ul> <p>as connecting with transportation resources.</p> <ul> <li>Assist and ensure individuals have primary care appointments, appropriate specialist </li></ul> <p>appointments and any necessary testing scheduled to close care gaps and reduce ED/</p> <p>hospital utilization, working closely with L.A. Care provider teams.</p> <ul> <li>Document member and non-member information and interactions following </li></ul> <p>documentation best-practices.</p> <ul> <li>Complete proper escalation and triage protocol as needed. </li><li>Support with existing relationships and development with community-based </li></ul> <p>organizations.</p> <p>Position Expectations:</p> <ul> <li>Be committed to the mission of L.A. Care. </li><li>Complete required training to align with role competencies. </li><li>Behave in a professional manner and consistently demonstrate and promote the </li></ul> <p>values of respect, honesty, and dignity for the member, families, and the health care</p> <p>team.</p> <ul> <li>Work in-person and onsite during business operating hours. </li><li>Accommodate flexible schedule with the potential to work evenings and weekend. </li><li>Committed to the constant pursuit of excellence and teamwork in improving the care </li></ul> <p>of the member and families in the community.</p> <ul> <li>Be punctual for scheduled work. </li><li>Maintain confidentiality and protect the program by abiding by laws and principles </li></ul> <p>related to confidentiality</p> <p>Qualifications:</p> <ul> <li>Valid California Driver's License </li><li>High school graduate or equivalent required; Associates Degree with areas of focus </li></ul> <p>in social work, public health, behavioral health, administration or related field</p> <p>preferred.</p> <ul> <li>Basic computer skills required. </li><li>Written and oral fluency in English is required; Spanish fluency may be required for </li></ul> <p>some locations; Depending on location of placement, additional languages may be</p> <p>preferred:</p> <p>o East L.A. - Korean and/or Tagalog</p> <p>o El Monte - Vietnamese</p> <p>o West L.A. - Persian</p> <p>o Panorama City - Tagalog and/or Armenian</p> <ul> <li>Experience working in a multi-cultural and community-based setting for at least 1 to 2 </li></ul> <p>years preferred.</p> <p>What We Do</p> <p>Established in 1997, L.A.
