<p>Qualifications:</p> <p>Bachelor's degree in a related field: Human Services and Social Work</p> <p>Travel Requirements: 50-75% (Regional/Local)</p> <p>Schedule: 40 hrs per week, Occasional Weekend / Evening hours</p> <p>Duties & Responsibilities:</p> <p>The Prevention Specialist II is to assist in ensuring effective implementation of prevention services within serving counties of HopeHealth, Inc., according to community and agency needs.</p> <ul> <li>Knowledgeable/oriented of grant logistics </li><li>Perform data entry, record keeping, DPH surveillance reporting, Provide Enterprise and documentation and quality assurance for testing inventory </li><li>Conducts HIV/Hepatitis C, substance abuse screenings and facilitates DEBI interventions </li><li>Provide condom distribution (monthly/quarterly) in accordance to community needs </li><li>Make appropriate referrals for clients to obtain adequate and relevant services as identified in their assessments </li><li>Assist/Participate in community events to include: community/church health fairs, DAODAS sites, housing authorities, detention centers, college campuses, health facilities, etc. </li><li>Provide/Assist with services to expedite linkage to care for clients new to the agency, i.e. obtaining proof of eligibility, proof of HIV labwork, identification card, transportation to obtain income documentation </li><li>HIV testing of partners, family members and appointment companions of clients returning to care. </li><li>Documents and ensures confidentiality as well as data security while on, community and during transportation to/from events, etc. </li><li>Adhere to all quality assurance procedures, policies, and guidelines within grant, agency and health department </li><li>Participates and adhere in team activities, meetings, trainings, events etc. This includes quality improvement meetings, supervisory meetings, and agency-specific meetings as indicated </li><li>Travel to trainings, partner meetings and testing locations </li><li>Attends DPH and CDC-required trainings, webinars and conferences </li><li>Performs other job duties as requested by Supervisor or Site Manager </li></ul> <p>Skills & Knowlege Required:</p> <p>Excellent communication and organization skills, Knowledge of HIV/Hepatitis C</p> <p>and STD's, and knowledge of Microsoft XP Tools</p>
