<p>Employment Type:</p> <p>Part time</p> <p>Shift:</p> <p>Description:</p> <p>The Contignent Hospice Medical Director at Trinity Health at Home (THAH) oversees medical direction for patient and family services, collaborates with the Executive Director and clinical team, and supports Provider Relations and CPI Committee activities. They also lead the QAPI Committee.</p> <p>Supervision Exercised</p> <p>THAH Hospice and Palliative Care physicians report to this role.</p> <p>Minimum Qualifications</p> <ul> <li>Licensed physician (M.D. or D.O.) </li><li>Valid driver's license and access to a vehicle </li><li>Ability to assess, plan, evaluate, and analyze patient care </li><li>Experience in leading patient-centered care </li><li>Competence in verbal, written, and word processing communication </li><li>Time management and autonomous functioning </li><li>Commitment to THAH mission and ethics </li></ul> <p>Preferred Qualifications</p> <ul> <li>Board Certification in Hospice & Palliative Medicine or willingness to obtain </li><li>Credentialing at hospitals serving hospice patients or willingness to obtain </li></ul> <p>Essential Functions</p> <ul> <li>Oversee and evaluate medical services quality </li><li>Review and create medical policies and procedures </li><li>Maintain a network of physicians for hospice needs </li><li>Provide 24-hour on-call availability </li><li>Mentor physicians and promote hospice services </li><li>Review medical care in home or inpatient settings </li><li>Consult with the patient's physician and hospice team </li><li>Determine hospice admission level of care </li><li>Certify and recertify patient life expectancy </li><li>Participate in Interdisciplinary Team meetings </li><li>Ensure physician coverage for routine and emergency situations </li><li>Develop educational programs for community physicians </li><li>Attend Executive, QAPI, and Ethics Committee meetings </li><li>Lead the Hospice Interdisciplinary Team </li><li>Establish and update patient care plans </li><li>Assume direct medical care or assign physicians </li><li>Manage inpatient hospice care if needed </li><li>Participate in on-call medical coverage </li><li>Perform face-to-face visits for symptom management and certifications </li><li>Adhere to confidentiality practices </li></ul> <p>Ministry/Facility Information: MercyOne Home Health Care and Hospice Agency, will cover Des Moines, Monticello and North Iowa (a Trinity Health Company), provides compassionate, exceptional care in our patient's home.</p> <p>Apply today!</p> <p>Our Commitment to Diversity and Inclusion</p> <p>Trinity Health is one of the largest not-for-profit, Catholic healthcare systems in the nation.
