<p>RN Case Manager - Case Management - Full Time Days 7:30 am - 4:00 pm and rotating weekends</p> <ul> <li>Department: Case Management </li><li>Schedule: Full Time Days 7 am - 4:00 pm Monday - Friday and rotating weekends </li><li>Hospital: Henry Ford Health Genesys </li><li>Location: Grand Blanc, Michigan </li></ul> <p>GENERAL SUMMARY</p> <ul> <li>Creates plan for care across the continuum, integrating patient/family preferences and values. </li><li>Monitors patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records. </li><li>Advocates for resources and removal of barriers. </li><li>Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan. </li><li>Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care. </li></ul> <p>Education/Experience Requirements</p> <ul> <li>Licensed Registered Nurse credentialed from the Michigan Board of Nursing required. </li><li>Professional Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association preferred </li></ul> <p>Preferences:</p> <ul> <li>RN Case Manager/Navigator experience helpful </li><li>Hospital experience helpful </li><li>Acute Care Nursing Experience 2-3 years highly preferred. </li><li>Multitasking, Good Communication and Computer Literacy highly preferred. </li></ul> <p>Additional Information</p> <ul> <li>Organization: Henry Ford Health Genesys Hospital </li><li>Department: Case Management 001 </li><li>Shift: Day Job </li><li>Union Code: Not Applicable </li></ul>
