<p>Employment Type:</p> <p>Full time</p> <p>Shift:</p> <p>Description:</p> <p>Occupational Therapist</p> <p>Location: Saint Francis LIFE College Avenue, Newark, DE</p> <p>Status: Full time</p> <p>Shift: Friday, 8:00 AM - 4:30 PM (No holidays/weekends/on call)</p> <p>Position Purpose:</p> <p>Provide occupational therapy evaluation and treatment for participants in PACE programs, nursing homes, or in-home settings.</p> <p>Position Details:</p> <p>This position will work both in the clinic and complete home care assignments as well. Shift time could also flex to 830 - 5 pm if desired.</p> <p>What you will do:</p> <ul> <li> <p>Perform in-person comprehensive occupational therapy assessment for initial acceptance, including problem identification and care planning.</p> </li><li> <p>Conduct assessment and coordination of home modification program including installation of equipment and other changes to support independence and safety.</p> </li><li> <p>Provide treatment to participants and implement occupational therapy procedures consistent in achieving identified goals, grade activities to facilitate goals, and confer with the Interdisciplinary Team on participants' care issues.</p> </li><li> <p>Assist in the ordering, inventory, distribution and maintenance of durable medical equipment to participants.</p> </li><li> <p>Carry out accepted principles and practices of occupational therapy for prevention of functional decline, sensory and cognitive limitations, increase maintenance of strength and endurance, and provides treatment to increase self-care skills.</p> </li><li> <p>Train family members, other caregivers, and staff in methods to maximize safety and functional abilities in performing Activity of Daily Living (ADL) skills.</p> </li><li> <p>Evaluate charts and reports on participants' progress, document occupational therapy notes in the participants' medical record according to the charting requirements.</p> </li><li> <p>Direct, supervise, perform initial and annual competencies on, and assist department manager with evaluating the performance of the Certified Occupational Therapy Assistant.</p> </li><li></li></ul> <p>Minimum Qualifications:</p> <ul> <li> <p>Bachelor's degree in occupational therapy from an accredited program.</p> </li><li> <p>Current, unencumbered, active occupational therapy license in the state of PACE Organization.</p> </li><li> <p>Two (2) years of appropriate experience as an occupational therapist preferred.</p> </li><li> <p>Valid driver's license and use of personal vehicle</p> </li><li> <p>Superior written and verbal communication skills.</p> </li><li> <p>Proficiency with MS Office.</p> </li><li> <p>Must manage conflict well, have high decision-making quality and flexibility to adapt to ongoing change.</p> </li><li> <p>Ability and skills to plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures of the organization in line with our mission, vision, and philosophy.</p> </li><li> <p>Position will require 10% of travel within the Centers geographic region due to the number of locations being supported.</p> </li></ul> <p>Position Highlights and Benefits:</p> <ul> <li> <p>Comprehensive benefit including 1st Day medical coverage, dental, vision, paid time off, 403B and educational assistance.</p> </li><li> <p>Access to daily pay and employee referral incentives.</p> </li><li> <p>Supportive environment with a patient-centered focus.</p> </li><li> <p>Opportunities for professional development.</p> </li></ul> <p>Ministry/Facility Information</p> <p>Saint Francis LIFE College Avenue is a part of Trinity Health PACE and provides high-quality care to seniors in the communities we serve.
