<p>Overview</p> <p>Pay Rate: $21.63/hour</p> <p>Are you passionate about making a difference in the lives of youth and families? We are seeking a dedicated Case Manager I - Family Support to join our team, where you'll provide crucial one-on-one support to parents and caregivers in a community setting. This role involves connecting families with resources, offering coaching, and helping build collaborative relationships with schools, service providers, and other community-based supports.</p> <p>Responsibilities</p> <ul> <li>Provide family coaching, support, and resources to help connect families with their children's care team, including schools, counselors, and healthcare providers. </li><li>Develop and implement care plans, provide advocacy, and assist with service coordination and resource development. </li><li>Maintain a caseload, ensuring documentation and service delivery are timely and meet best practice standards. </li><li>Facilitate support groups, maintain the Family Resource Center (FRC) calendar, and engage volunteers for specialized groups. </li><li>Collaborate with community partners and attend meetings and trainings to continue personal and professional development. </li></ul> <p>Qualifications</p> <ul> <li>High school diploma or GED required. </li><li>1-3 years of related experience. </li><li>Valid driver's license and reliable personal transportation. </li><li>Experience or knowledge in youth and family casework, wrap-around care, and community resources. </li><li>Ability to build effective relationships with clients, families, and providers. </li><li>Strong communication, organizational, and problem-solving skills. </li><li>Familiarity with child welfare, mental health, education, or juvenile justice systems is a plus. </li></ul>
