<p>Planned Parenthood Northern California (PPNorCal) is seeking a Community Health Worker for our Vallejo and Fairfield Health Centers. This role is full-time, 35 hours per week. PPNorCal provides a comprehensive benefits package including 100% employer paid medical insurance for the employee, 50% employer paid medical insurance for dependent children, dental and vision coverage, employer paid life and long-term disability insurance, 10 paid holidays, and a 403(b) retirement plan with up to 4% employer match.</p> <p>This position is currently represented by SEIU Local 1021.</p> <p>APPLICATION INSTRUCTIONS: After review of your application, resume and qualifications, you may be invited to the next phase of the selection process, which includes a pre-recorded video interview, responding to screening questions. If you are being invited to do the video interview, you will receive an email and text with the video platform link and will have 5 days to complete this video interview.</p> <p>SUMMARY</p> <p>The Community Health Worker (CHW) is a liaison between health, social services and the community and facilitates access to services to improve the quality and cultural competence of services delivered. The CHW has a close understanding of the community served in the delivery of health-related services through either working directly with providers or their partner organizations. CHWs build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, patient navigation, social support, and advocacy.</p> <p>Essential Duties and Responsibilities may include the following. Other duties may be assigned.</p> <ul> <li>Function as an integrated member of the Vallejo/Fairfield health center staff. </li><li>Trained in specific Reproductive Health Specialist (Medical Assistant equivalent) functions within the Vallejo/Fairfield health centers. </li><li>Identify Community Based Organizations (CBOs) to collaborate with to meet needs related to sexual and reproductive health resources and education; maintain relationships with CBOs </li><li>Build and maintain collaborative and positive relationships with the community, health center staff, and partner organizations. </li><li>As a member of the health center team, provide education to individuals or groups on topics related to health and health promotion, which may include sexual and reproductive health, communicable disease prevention, testing, vaccination, and support services. </li><li>Collaborate with CBOs as appropriate to deliver bilingual (Spanish/English) culturally sensitive sexual and reproductive health education and information </li><li>Provide information and referrals to community resources necessary to improve health or address health-related social needs, including, but not limited to, food and nutrition, housing security, economic support, and legal services. </li><li>Assist and/or navigate patients in accessing health-related services and community resources, such as MyChart set up, follow-up appointments, provider eligibility and insurance coverage options. </li><li>Collect data and information as required by the evaluation plan and processes. Provide </li></ul> <p>feedback to evaluation team and program management on evaluation process. Experience with data tracking/survey tools preferred.</p> <ul> <li>Use assigned technology to complete job functions effectively. </li><li>Maintain and document confidential information. </li><li>Submit regular report on program implementation, data of individuals receiving assistance, and client stories sharing highlights, challenges, and recommendations </li><li>Ability to collaborate with supervisor on grant-specific narrative/writing in support of grant deliverables </li><li>Provide ongoing support and services for patients as needed. </li><li>Engage, motivate, and advocate community members to build knowledge and skills for self-directed change and community development. </li><li>Perform other job duties as required by manager/supervisor. </li><li>Actively participate in and support the Diversity, Equity, and Inclusion initiatives of PPNorCal. </li></ul> <p>MINIMUM QUALIFICATIONS</p> <ul> <li>Understands and is part of the community being served. </li><li>Experience in community work, education, health care, or related field preferred </li><li>Demonstrated self-awareness in terms of understanding one's own race, culture, identities, biases, power, privileges, and stereotypes. </li><li>An understanding of the diverse academic, socioeconomic, cultural, disability, gender identity, sexual orientation, and ethnic backgrounds present in the community. </li><li>Ability to communicate easily with others, including demonstrating active listening skills. </li><li>Sensitivity to populations most highly affected by homelessness and other at-risk populations. </li><li>Read, write and speak English language. </li><li>Fluency in Spanish, required. </li></ul> <p>SUPERVISORY RESPONSIBILITIES</p> <p>This position has no supervisory responsibilities.</p> <p>QUALIFICATIONS</p> <p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Bachelors level work completion preferred; co-concurring undergraduate education acceptable. Experience in a community clinic and in reproductive health care is desirable.</p> <p>LANGUAGE SKILLS</p> <p>Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the PPNorCal organization.</p> <p>MATHEMATICAL SKILLS</p> <p>Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.</p> <p>REASONING ABILITY</p> <p>Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.</p> <p>COMPUTER SKILLS</p> <p>To perform this job successfully, an individual should have knowledge of Spreadsheet software and Word Processing software. This new law requires employers with 15 or more employees to include a pay range in job postings. The pay range reflects the compensation that a new employee can expect to receive if offered employment. Employees with more experience, education, and other qualifying factors may be compensated at a higher rate of pay.
