<p>The Licensed Practical Nurse is accountable for adhering to established policies and procedures and for providing patient care. Assesses, plans, Implements and evaluates care based on age-specific components. </li><li>Administers vaccines, places laboratory orders, and other medications as per policy and within scope of practice. </li><li>Demonstrates understanding of age and population specific characteristics that affect the healthcare needs of the veteran. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. </li><li>Bases care on the patient's goals, provides patient and family health education including self-management, prevention, and wellness. Conducts patient education on blood glucose testing and glucometers, blood pressure monitoring, and provides patient educational material for chronic diseases. </li><li>Promptly recognizes urgent or emergent patient care situations, seeks assistance, and effectively intervenes as directed when appropriate. </li><li>Interacts and communicates appropriately with patients and family members, demonstrating professional behavior (i.e respect others, collegiality, ability to accept constructive criticism), effective communication and interpersonal skills. </li><li>Adheres to principles of infection control, safety, and universal precautions. Possess knowledge of proper handling and pre-cleaning of reusable medical equipment.
