<p>The Licensed Practical Nurse is accountable for adhering to established policies and procedures and for providing patient care. The nurse makes observations and thoroughly documents care and the patient's response; and appropriately seeks assistance and guidance by informing Registered Nurse or medical provider of changes in the patient's condition requiring higher-level intervention.</p> <p>VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4</p> <p>Total Rewards of a Allied Health Professional</p> <p>Major duties/functions include, but are not limited to:</p> <ul> <li>Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care, including clinical reminders, health screenings, vital signs, and education, seeing patients face-to-face for acute care needs, management of flow in clinic, point of contact for Primary care needs for internal/external customers. </li><li>Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Assesses, plans, Implements and evaluates care based on age-specific components. </li><li>Administers vaccines, places laboratory orders, and other medications as per policy and within scope of practice. </li><li>Demonstrates understanding of age and population specific characteristics that affect the healthcare needs of the veteran. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. </li><li>Bases care on the patient's goals, provides patient and family health education including self-management, prevention, and wellness. Conducts patient education on blood glucose testing and glucometers, blood pressure monitoring, and provides patient educational material for chronic diseases. </li><li>Promptly recognizes urgent or emergent patient care situations, seeks assistance, and effectively intervenes as directed when appropriate. </li><li>Interacts and communicates appropriately with patients and family members, demonstrating professional behavior (i.e respect others, collegiality, ability to accept constructive criticism), effective communication and interpersonal skills. </li><li>Adheres to principles of infection control, safety, and universal precautions. Possess knowledge of proper handling and pre-cleaning of reusable medical equipment. Demonstrates competence utilizing special equipment needed to perform job duties. </li><li>Follows policy and procedures related to fire and disaster drills, safety and security measures. </li><li>Assists in the orientation and competency validation of students, peers, and clerical staff. </li><li>Maintains confidentiality of electronic, written, and/or verbal patient information. Maintains a work environment that honors diversity. </li><li>Demonstrates active collaboration and communication with the PACT Core Team (Patient, Primary Care Provider, RN Care Manager, and MSA) and Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient). </li><li>All other duties as assigned, pertaining to this occupation, to meet the needs of the Agency, Department, Facility, and/or Service. </li></ul> <p>Work Schedule: Full Time, Monday thru Friday; 7:00AM - 3:30PM</p> <p>Recruitment Incentive (Sign-on Bonus): Not Authorized</p> <p>Permanent Change of Station (Relocation Assistance): Not Authorized</p> <p>Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).</p> <p>Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)</p> <p>Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.</p> <p>Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.</p> <p>Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.</p> <p>Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA</p> <p>Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)</p> <p>Telework: Not Available</p> <p>Virtual: This is not a virtual position.</p> <p>Functional Statement #: 000000</p> <p>Permanent Change of Station (PCS): Not Authorized</p>
