Individual Support Counselors implements individualized programs as specified in Individual Service Plans, provides for individuals health and welfare, and ensures general cleanliness and safety in individuals homes.</p> <ul> <li>Providing Individual community outings </li><li>Encourage rapport with local resources </li><li>Instruct and assist with meal preparation </li><li>Assistance with activities of daily living </li><li>Instruct and physically assist with housekeeping activities </li><li>Medical appointments </li><li>Administer and documentation of medication and/or monitor medical health </li><li>Assist with financial management </li><li>Ensure home is safe </li></ul> <p>What are Community Outings?</p> <p>As a Individual Support provider you will provide opportunities for individuals to integrate into the community and enjoy outings that interest them.
