This includes identifying eligible offenders, developing and tracking a discharge plan for each offender assigned to the caseload. The discharge plan should include all necessary referrals/assessments to ensure a continuum of care upon release. This position is a lead worker and will be assigned cases that require more intense intervention. In addition, this role will be required to provide data for the team as requested.</p> <p>Job Responsibilities may include but are not limited to the following:</p> <p>Identifying Offenders:</p> <ul> <li>Work with the Reentry Coordinator to identify offenders housed at LCF who are eligible for services due to disability (SPMI, DD, LD, elderly), who require discharge planning services and are approximately six months from release. Work with unit team and behavioral health staff to establish a referral mechanism to assist in identifying offenders requiring/eligible for these services. Identify inmates eligible for the K-Shop program. Send the referral form and release of information for potential participants to the appropriate K-Shop staff. </li></ul> <p>Managing Caseload:</p> <ul> <li>Contact the unit team counselor to engage the offender in the discharge planning process. Identify strategies to engage the offenders in the process and keep them engaged until release. Secure necessary information about the offenders' disability to determine what the discharge plan needs to include. Participate in Multi-Disciplinary Team (MDT) meetings as requested by unit team. Initiate MDT meetings as necessary to address issues with the discharge plan. Develop and maintain a system to track caseload of offenders and tasks that have been and need to be completed. Establish contact with the assigned parole officer prior to release. Make sure they are aware of the discharge plan established to ensure a continuum of care upon release. </li></ul> <p>Discharge Plan:</p> <ul> <li> <p>Develop a discharge plan goal in the Case Plan for every offender within 30 days of being assigned to caseload. Action Steps should be created to include (not limited to):</p> </li><li> <p>RADAC referral to identify/address substance abuse issues</p> </li><li> <p>Schedule intake appointment at Community Mental Health Center (CMHC)</p> </li><li> <p>Department of Aging referral for Nursing Facility or Nursing Facility for Mental Health (NFMH) placements</p> </li><li> <p>Prepare and submit benefits applications as necessary (Social Security - SSI/SSDI, Medicaid, MediKan, Medicare, General Assistance, TAF, Food Stamps, etc…)</p> </li><li> <p>Medication management/plans</p> </li><li> <p>Any other service/referral to address the offender's disability</p> </li><li> <p>The discharge plan goal & action steps should be updated regularly and shared with the offender, unit team counselor and any other necessary staff (behavioral health, medical, etc.). Contact should be made and documented (in TOADS) at least once a month with every month. Work with Corizon staff to ensure that medication is in each offender's hand prior to release.</p> </li></ul> <p>Relationships and Information-Sharing:</p> <ul> <li>Obtain appropriate releases for information-sharing within and without the facility and with community providers. Working closely with existing services (KDOC staff, KSHOP and parole offices), establish relationships with community providers to establish methods for processing applications for benefits pre-release and ensure a continuum of care upon release. Serve as the liaison for the K-Shop program. This includes; entering callouts as requested, notifying K-Shop staff if participants are in crisis or a change in housing/status, assist in completing the SSI/Medicaid applications, and participating in K-Shop meetings as requested by K-Shop staff. </li></ul> <p>Maintain data and information related to processes and impact, and provide the information to the statewide reentry team, as requested by the Reentry Director.</p> <p>Other duties as assigned.</p> <p>Qualifications</p> <p>Minimum Qualifications:</p> <p>One year experience in planning, implementing and monitoring activities relevant to the agency's programs. Education may be substituted for experience as determined relevant by the agency.</p> <p>Preferred Qualifications:</p> <p>Experience with offenders and/or special needs populations.</p> <p>Case Management and/or Discharge Planning experience.</p> <p>Bachelor's degree with major coursework in criminal justice, the social sciences, or the behavioral sciences, and one year of experience working with offenders in corrections, parole, or law enforcement.</p> <p>Experience with special needs populations.</p> <p>Case Management and/or Discharge Planning experience.</p> <p>Post-Offer, Pre-employment Requirements: Must possess and maintain a valid driver's license.
