Come find your next professional home at PrimaryOne Health!</p> <p>WORK FOR US!</p> <p>PrimaryOne Health is currently seeking a Clinical Lead for our East 17th Health center.</p> <p>Essential Job Responsibilities:</p> <ul> <li> <p>Provides professional patient care for adults, prenatal and pediatric patients to the full scope of their license as indicated in a Federally Qualified Health Center (FQHC). Including efficient collection of patient history, vital signs, injections, vena-puncture, EKG, outpatient procedures and patient education.</p> </li><li> <p>Performs basic chart audits and educates staff on potential improvement areas and reports any clinical error if identified.</p> </li><li> <p>Works together with Operations Manager and Lead Provider to establish and maintain an effective and collaborative leadership team at the site level.</p> </li><li> <p>Serves as clinical resource for site support staff.</p> </li><li> <p>Prioritizes and collaborates with the Operations Manager to direct support staff workflow.</p> </li><li> <p>Monitors and maintains undocumented vaccine, medication, lab and procedure reports.</p> </li><li> <p>Initiates and maintains daily huddles with staff and documents agenda, participants and action item and maintains huddle book or other form of communication for all to review.</p> </li><li> <p>Along with Operations Manager and lead provider, Clinical Lead assures that all quality measures identified by PrimaryOne Health are being met.</p> </li><li> <p>Review patient charts daily for accuracy and thoroughness (meds and procedures documented).</p> </li><li> <p>Under guidance and direction from Clinical Manager, educate and train MAs and LPNs on an on-going basis to improve knowledge.</p> </li><li> <p>Maintains communication with patients concerning treatment progress and diagnostic reports. Coordinates care with other health services teams as appropriate.</p> </li><li> <p>Oversees immunization inventory and accountability and ensure all expired medications are disposed of according to guidelines at the discretion of the Clinical Process Supervisor.</p> </li><li> <p>Participates in clinical evaluations for new and established employees.</p> </li><li> <p>Identifies and coordinates referrals to community resources regarding patient needs.</p> </li><li> <p>Documents all professional activities, contacts and observations promptly and appropriately.</p> </li><li> <p>Attends internal and community meetings as assigned and participates in Quality Improvement activities as assigned.</p> </li><li> <p>After hours / Saturday hours as required by organizational need.</p> </li><li> <p>Attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors, and patients.</p> </li><li> <p>Provides oversight of EHR inbox and documents weekly report of inbox status with detailed explanation of any messages/documents falling outside of the guidelines.</p> </li><li> <p>Will collaborate with the Operations Manager to complete inventory and stock, manage the ordering list and initiate orders for clinical supplies.</p> </li><li> <p>At the direction of the Clinical Manager and Operations Manager will oversee the monitoring, tracking and documentation of CLIA waived quality assurance testing and sterilization monitoring (ex: urinalysis, A1C, autoclave, etc.).</p> </li><li> <p>Oversee and maintain crash cart monthly quality and inventory monitoring.</p> </li><li> <p>Occasional travel required.</p> </li><li> <p>Other duties or special projects as assigned.</p> </li></ul> <p>Successful candidates must possess High School Diploma or equivalent; current /valid licensure as a Practical Nurse in the State of Ohio/ or current documentation of completion of a Medical Assistant program or its equivalent.
