<p>As a Physician, you will be responsible for coordination of proactive care for patients with chronic conditions as well as identifying wellness and/or preventative service for clinical patients. This position will work closely with the clinical staff and will report to the Site Medical Director.</p> <p>JOB RESPONSIBILITIES:</p> <ul> <li> <p>Utilizes appropriate and effective patient care plans which includes adequate diagnostic procedures, therapies, use of consultation and referrals, and outcomes.</p> </li><li> <p>Manages patients to assure continuity of care which includes adequate scheduling and tracking of return visits, completion of treatment plans, and follow-up of chronic problems, abnormal tests, and no show appointments.</p> </li><li> <p>Provides whole-person care which includes appropriate</p> </li><li> <p>referrals to the social worker, nutritionist, and mental health professionals.</p> </li><li> <p>integration of biologic, social, psychological and environmental issues.</p> </li><li> <p>Maintains adequate, concise, and legible documentation of patient care.</p> </li><li> <p>Provides and documents appropriate periodic health examinations. (i.e. preschool vision exams, serum cholesterol, pap smears, etc.)</p> </li><li> <p>Educates patients about their diagnosis, treatment, prognosis, medications, preventive care, etc.</p> </li><li> <p>Utilizes appropriate infection control measures, including universal precautions.</p> </li><li> <p>Supervises advance practice nurses in Clinic when assigned.</p> </li><li> <p>Provides consultation and prescription signatures to advance practice nurses when needed.</p> </li><li> <p>Oversees residents in the management of hospital patients.</p> </li><li> <p>Writes daily progress notes in hospital charts.</p> </li><li> <p>Takes phone calls about hospital patients.</p> </li><li> <p>Participate in assigned Quality Assurance/Management in Daily Work activities.</p> </li><li> <p>Regularly attends QA/TQM as required.</p> </li><li> <p>Participates in the physician performance audit as assigned.</p> </li><li> <p>Assists in the development and maintenance of HealthLinc practice guidelines (protocols) as assigned.</p> </li><li> <p>Participates in Total Quality Management (TQM) activities.</p> </li><li> <p>Attends Clinics Provider Group Meetings and hospital section meeting as scheduled.</p> </li><li> <p>Attends required in-service meetings.</p> </li><li> <p>Makes presentations in the community when requested.</p> </li><li> <p>Teaches students and residents as assigned.</p> </li><li> <p>All HealthLinc staff is committed to engage in quality improvement initiatives that align with and support Patient-Centered Medical Home (PCMH).</p> </li><li> <p>Performs other duties as assigned.</p> </li></ul>
